血浆转铁蛋白、P-选择素对老年急性胰腺炎早期体液复苏疗效评估价值

摘　　要：目的 研究血浆转铁蛋白、P-选择素对老年急性胰腺炎早期体液复苏疗效评估的价值。方法 收集2019年1月—2021年1月本院收治的113例急性胰腺炎患者为观察组,同期在本院体检健康者113例为对照组,对比观察组与对照组血浆转铁蛋白、P-选择素水平,通过t检验和Spearman检验分析血浆转铁蛋白、P-选择素水平与患者病情程度、疗效、预后的关系,通过受试者工作特征曲线(ROC)分析转铁蛋白、P-选择素对老年急性胰腺炎早期体液复苏疗效评估价值。结果 与对照组比较,观察组血浆转铁蛋白水平明显下降[(2.97±0.73)g/L vs(1.02±0.30)g/L](t=7.850,P<0.001);与对照组比较,观察组血浆P-选择素水平明显上升[(9.48±2.41)μg/mL vs(57.74±7.15)μg/mL](t=-67.991,P<0.001)。与轻中度、治疗有效、生存患者比较,重度、治疗无效、死亡患者血浆转铁蛋白水平明显下降[(1.32±0.46)、(2.89±0.75)、(2.93±0.69)g/Lvs(0.84±0.19)、(1.05±0.37)、(0.98±0.33)g/L](t=7.850、17.402、20.274,P<0.001),血浆P-选择素水平明显上升[(52.11±5.27)、(9.56±2.38)、(9.49±2.43)μg/mL vs(72.18±8.86)、(58.17±7.32)、(61.12±7.24)μg/mL](t=-12.156、-37.343、-40.046,P<0.001)。转铁蛋白水平与急性胰腺炎患者病情程度、疗效、预后均相关(r=-0.740、0.823、0.765,P<0.001),P-选择素水平亦与其相关(r=0.842、-0.778、-0.648,P<0.001)。血浆转铁蛋白、P-选择素联合评估患者疗效的AUC值(0.874)>P-选择素(0.788)>转铁蛋白(0.726)。结论老年急性胰腺炎患者血浆中转铁蛋白呈低表达水平,P-选择素呈高表达水平,2项指标水平与老年急性胰腺炎患者病情程度、疗效及预后情况关联密切,对其早期体液复苏疗效有较高的评估价值。Objective To study the value of plasma transferrin and P-selectin in the evaluation of early humoral resuscitation in elderly patients with acute pancreatitis.Methods A total of 113 patients with acute pancreatitis treated in our hospital from January 2019 to January 2021 were selected as an observation group, and 113 healthy persons who underwent physical examination in our hospital during the same period were selected as a control group. The levels of plasma transferrin and P-selectin in the observation group and the control group were compared. The relationships between the levels of plasma transferrin and P-selectin and the severity, efficacy, and prognosis of patients were analyzed by t-test and Spearman test. The evaluation value of transferrin and P-selectin on the efficacy of early humoral resuscitation in elderly patients with acute pancreatitis was analyzed by the receiver operating characteristic curve(ROC).Results Compared with the control group, the level of plasma transferrin in the observation group decreased significantly [(2.97 ± 0.73) g/L vs(1.02 ± 0.30) g/L](t = 7.850, P < 0.001);Compared with the control group, the plasma P-selectin level in the observation group increased significantly [(9.48 ± 2.41) μg/mL vs(57.74±7.15)μg/mL](t =-67.991, P < 0.001). Compared with mild to moderate, effective and surviving patients, the plasma transferrin level of severe, ineffective and dead patients decreased significantly [(1.32 ±0.46),(2.89 ± 0.75),(2.93 ± 0.69) g/L vs(0.84 ± 0.19),(1.05 ± 0.37),(0.98 ± 0.33) g/L](t = 7.850, 17.402, 20.274, all P <0.001), but the plasma P-selectin levels increased significantly [(52.11 ± 5.27),(9.56 ± 2.38),(9.49 ± 2.43) μg/mL vs(72.18 ±8.86),(58.17 ± 7.32),(61.12 ± 7.24) μg/mL](t =-12.156,-37.343,-40.046, all P < 0.001).
