四黄汤加味水疗对痔疮术后疼痛、水肿及创面菌落的影响分析

摘　　要：目的:探讨四黄汤加味水疗对痔疮术后疼痛、水肿及创面菌落等康复指标的影响。方法:选取2020年1月—2021年12月厦门市同安区中医医院收治的80例痔疮患者作为研究对象,所有患者均接受手术治疗。使用随机数表法将患者分成两组,各40例。对照组术后常规高锰酸钾水疗治疗,观察组术后四黄汤加味水疗治疗。比较两组术后疼痛程度、愈合情况、中医症候积分、治疗效果、生活质量及创面菌落情况。结果:治疗后,两组视觉模拟评分法(VAS)、水肿、出血、渗液、坠胀评分均低于治疗前,且观察组均低于对照组(P<0.05)。观察组水肿消失、出血消失、渗液消失、坠胀消失、创口愈合时间均早于对照组(P<0.05)。观察组治疗总有效率为95.00%,高于对照组的75.00%(P<0.05)。治疗后,两组生活质量评分较治疗前均升高,观察组高于对照组(P<0.05)。两组治疗后7 d创面菌落检查中均未分离出致病菌。结论:四黄汤加味水疗在痔疮术后治疗中作用明显,可有效缓解术后疼痛,使水肿、出血、渗液等症状快速消失,继而缩短患者术后康复时间,有利于改善生活质量,且该方案依然可有效消灭致病菌,降低微生物感染风险。Objective:To explore the effect of Sihuang Decoction modified hydrotherapy on postoperative pain,edema and wound bacterial colony of hemorrhoids.Method:A total of 80 patients with hemorrhoids who treated in Xiamen Tong’an District Hospital of Traditional Chinese Medicine from January 2020 to December 2021 were selected as the study subjects,all patients were given surgical treatment.The patients were divided into two groups by random number table method,40 cases in each group.After operation,the control group was given routine Potassium Permanganate hydrotherapy,and the observation group was given Sihuang Decoction modified hydrotherapy.The degree of postoperative pain,healing,traditional Chinese medicine symptom score,therapeutic effect,quality of life and wound bacterial colony were compared between the two groups.Result:After treatment,the scores of visual analogue scale(VAS),edema,bleeding,exudation and swelling in two groups were lower than those before treatment,and those in the observation group were lower than those in the control group(P<0.05).The times of edema disappearance,hemorrhage disappearance,exudation disappearance,bulge disappearance and wound healing in the observation group were earlier than those in the control group(P<0.05).The total effective rate of the observation group was 95.00%,which was higher than 75.00%of the control group(P<0.05).After treatment,the scores of quality of life in two groups increased compared with those before treatment,the score in the observation group was higher than that in the control group(P<0.05).No pathogenic bacteria were isolated from the wound in two groups at 7 d after treatment.Conclusion:Sihuang Decoction modified hydrotherapy plays an obvious role in the post-operative treatment of hemorrhoids,which can effectively alleviate the postoperative pain of patients,make the symptoms of edema,bleeding and exudation disappear quickly,and then shorten the postoperative rehabilitation time,which is conducive to significantly improving the quality of life.More
