臭氧髓核溶解术联合粘连松解术治疗颈椎间盘源性疼痛的临床观察

摘　　要：目的:观察臭氧髓核溶解术联合粘连松解术在治疗颈椎间盘源性疼痛的临床疗效。方法:206例患者随机分为A组(100例,行颈椎间盘臭氧髓核溶解术)和B组(106例,行颈椎间盘臭氧髓核溶解术联合硬膜外前间隙粘连松解术)。在微创术中对疑似病变椎间盘先后进行经典椎间盘造影和臭氧椎间盘造影,分别观察疼痛反应及病理分型。在术前及术后第1天,出院后随访第1月和第6月进行视觉模拟疼痛评分(VAS),并用SPSS 13.0软件进行统计学处理。结果:在治疗后第1天、随访1个月及6个月时B组的总有效率、优良率明显高于A组。A组内随访6个月时的效果较术后第1天明显下降。结论:联合疗法可明显提高颈椎间盘源性的患者的临床疗效。Objective:To observe the clinical efficacy of ozonucleolysis combined with adhesiolysis in the treatment of cervical discogenic pain.Methods:Two hundred and six patients were randomly divided into group A(100 cases,underwent ozonucleolysis in cervical intervertebral disc)and group B(106 cases,underwent ozonucleolysis in cervical intervertebral disc combined with adhesiolysis in epidural prevertebral space).Classical discography and ozone discography were performed successively for suspected diseased intervertebral discs during minimally invasive surgery.Pain reaction and pathological typing were observed.Visual analog scale(VAS)score was performed preoperatively and on postoperative day 1,at 1 and 6 months followup after discharge and statistical processing was carried out using SPSS 13.0 software.Results:The overall response rate,excellent and good rate in group B on d1 after treatment and at the 1st and 6th months of follow-up were significantly higher than those in Group A.The efficacy of group A at the 6th month of follow-up was significantly decreased compared with that on d1 after surgery.Conclusion:The combination therapy can significantly improve the clinical outcome of patients with cervical discogenic pain.
