摘 要:目的 探讨二维斑点追踪技术(2D-STI)评价胆道闭锁(BA)患儿肝移植术前左心室收缩功能的临床价值。方法 选取肝功能失代偿期BA拟行肝移植手术患儿31例和正常对照组39例行常规超声心动图获得常规超声参数,2D-STI测得左心室18节段纵向应变(LS)、圆周应变(CS)以及整体纵向应变(GLS)和整体圆周应变(GCS),分析比较两组间各参数的差异。结果 (1)左心室各节段LS比较,BA组前间隔心尖段较对照组减低,差异有统计学意义(P<0.05);(2)BA组和对照组前侧壁的LS中间段<基底段,对照组前间隔和下侧壁的CS中间段<基底段,其余各室壁节段LS及CS均呈现心尖段>中间段>基底段的趋势。结论 2D-STI技术检测左室心肌应变能够评价BA患儿肝移植术前局部收缩功能早期受损,可为临床提供术前评估依据。Objective To investigate the clinical value of two-dimensional speckle tracking imaging(2 D-STI) in evaluating left ventricular systolic function in children with biliary atresia(BA) before liver transplantation. Methods A total of 31 children with BA in the period of hepatic decompensation for liver transplantation and 39 children in the normal control group were selected. Conventional echocardiography was performed to obtain the conventional ultrasound parameters. The longitudinal strain(LS), circumference strain(CS), global longitudinal strain(GLS) and global circumference strain(GCS) of the 18 segments in left ventricle were measured by 2 D-STI,and the differences of the parameters between the two groups were analyzed and compared. Results(1)The LS of the anterior septal apical segment in the BA group was reduced compared with the control group, and the difference was statistically significant(P<0.05).(2)The LS of middle segment in anterior lateral wall was less than the basal segment in the BA group and control group, and the CS of middle segment in anterior septum and inferior lateral wall was less than the basal segment in control group. In addition, the LS and CS showed a trend that apical segment>mid-segment>basal segment in the remaining ventricular wall segments. Conclusions The left ventricular myocardial segment strain detected by 2 D-STI can evaluate the early impairment of focal systolic function before liver transplantation in children with BA, and can provide a foundation for clinical preoperative evaluation.
