前列腺等离子剜切术与等离子电切术治疗大体积前列腺增生的效果比较

摘　　要：目的比较前列腺等离子剜切术(TUERP)与前列腺等离子电切术(TURP)治疗大体积前列腺增生(BPH)的效果。方法抽取南阳市第一人民医院2019年2月至2022年2月收治的大体积BPH患者210例,根据治疗方法分为对照组(150例)与观察组(60例),对照组予以TURP治疗,观察组予以TUERP治疗。比较两组的手术指标及尿动力学指标。结果与对照组比较,观察组手术时间、膀胱冲洗时间、尿管留置时间更短,手术出血量更少,前列腺膀胱组织切除量更多(P<0.05)。术前,两组尿动力学指标比较差异未见统计学意义(P>0.05);术后半年与对照组比较,观察组最大尿流更高、残余尿量更低(P<0.05)。结论相比TURP治疗大体积BPH,TUERP治疗可减少手术时间、冲洗时间、置管时间及出血量,切除病变组织更多,术后尿动力学指标改善更显著。Objective To compare the effect of plasma transurethral enucleation and resection of the prostate(TUERP)and plasma resection of the prostate(TURP)in the treatment of large-volume benign prostatic hyperplasia(BPH).Methods A total of 210 large-volume BPH patients admitted to Nanyang First People’s Hospital from February 2019 to February 2022 were selected for the research.According to treatment methods,they were divided into control group(150 cases)and observation group(60 cases).The control group was treated by TURP,and the observation group was treated by TUERP.The operation indexes,urodynamic indexes of the two groups were compared.Results Compared with the control group,the observation group had shorter operation time,shorter bladder washing time,shorter urinary tube retention time,less intraoperative bleeding,and more prostate and bladder tissue resection volune(P<0.05).Before operation,there was no significant difference in urodynamic indexes between the two groups(P>0.05).Compared with the control group,the observation group had higher maximum urine flow and lower residual urine volume half a year after operation(P<0.05).Conclusions Compared with TURP in the treatment of large-volume BPH,TUERP treatment can reduce the operation time,flushing time,catheterization time,moreover,and reduce the amount of blood loss,remove more diseased tissue,and cause more significant improvement in urodynamics after surgery.
