缺血性脑卒中患者的CT灌注成像特征及其脑血流量的影响因素

摘　　要：目的:探讨分析缺血性脑卒中患者CT灌注成像特征及其脑血流量的影响因素。方法:抽选2019年7月至2021年12月院内收治的缺血性脑卒中患者共计60例,均接受常规CT扫描与灌注成像,以患者健侧为对照区域,与患侧梗死区域、半暗带区域CBF、CBV、TTP及MTT进行对比,并探讨CBF影响因素。结果:梗死区域、半暗带区域CBF、CBV水平低于对照区域(P<0.05);梗死区域、半暗带区域MTT、TTP水平,相对对照区域偏高(P<0.05);梗死区域CBF、CBV水平,则相对低于半暗带区域(P<0.05);梗死区域MTT、TTP水平高于半暗带区域(P<0.05);相关性分析结果,患侧CBF值与CBV值为正相关,与TTP值、MTT值则为负相关(P<0.05);多因素分析结果,患侧CBF影响因素为MTT、TTP、缺血部位(P<0.05)。结论:CT灌注成像可以更好地掌握缺血性脑卒中患者血流参数,其血流灌注路径延长,同时速度缓慢;影响患者患侧CBF值的影响因素包括MTT、TTP、缺血部位,可为后续治疗提供支持。Objective:To investigate the CT perfusion imaging characteristics and the influencing factors of cerebral blood flow in patients with ischemic stroke.Methods:In this study,60 patients with ischemic stroke were selected and treated in the hospital.From July 2019 to December 2021,all patients received conventional CT scanning and perfusion imaging.The healthy side of the patient was taken as the control area,and compared with the affected side's infarct area,penumbra area CBF,CBV,TTP,and MTT,and discussed the influencing factors of CBF.Results:The levels of CBF and CBV in the infarct area and penumbra area were lower than those in the control area(P<0.05);the levels of MTT and TTP in the infarct area and penumbra area were higher than those in the control area(P<0.05);the levels of CBF and CBV in infarct area were lower than those in penumbra area(P<0.05);the levels of MTT and TTP in infarcted area were higher than those in penumbra(P<0.05);the results of correlation analysis showed that CBF was positively correlated with CBV,and negatively correlated with TTP and MTT(P<0.05);Multivariate analysis showed that the influencing factors of CBF were MTT,TTP,and ischemic site(P<0.05).Conclusion:CT perfusion imaging can better grasp the blood flow parameters of ischemic stroke patients,and its blood flow perfusion path is prolonged and the speed is slow;the influencing factors of CBF value on the affected side of patients include MTT,TTP,and ischemic site,which can provide support for follow-up treatment.
