摘 要:目的探讨白膜法分离血小板混合过滤法和血小板混合过滤法去除混合浓缩血小板中白细胞(white blood cell,WBC)对混合浓缩血小板残余WBC和血小板含量是否有影响。方法选择2021年6—12月泉州市中心血站符合文献要求的献血者所献全血400 mL,将血袋按1、2、3等顺序编号,编号是奇数血液生产的浓缩血小板用白膜法分离血小板混合过滤法去除WBC,编号是偶数血液生产的浓缩血小板用血小板混合过滤法去除WBC,计算这两种方法的WBC残余数和血小板计数(platelet count,PLT)的含量,同时计算残余WBC和PLT合格率,并进行比较分析。结果40袋白膜法分离血小板混合过滤法和40袋血小板混合过滤法去除WBC,其残余WBC平均值分别为(4.01±1.33)×10^(5)/袋和(1.63±0.91)×10^(5)/袋,前者WBC残余值是后者的2倍多,且差异有统计学意义(t=9.34,P<0.05)。两种过滤去WBC方法的PLT含量平均值分别为(3.47±1.49)×10^(11)/袋和(2.94±1.03)×10^(11)/袋,白膜法分离血小板混合过滤法PLT含量较高,但差异无统计学意义(t=1.85,P>0.05)。两种过滤方法去白细胞后WBC残余合格率分别为80.0%和97.5%,血小板混合过滤法WBC残余合格率97.5%,合格率明显较高,差异有统计学意义(P<0.05),两种去白细胞方法的PLT含量合格率分别为85.0%和75.0%,但差异无统计学意义(P>0.05)。结论血小板混合过滤法去除WBC比较好地平衡去除白细胞和保留血小板,因此建议采用血小板混合过滤法去除混合浓缩血小板WBC。Objective To investigate the effect of white membrane separation and platelet filtration on the removal of white blood cells(WBC)from mixed condensed platelets on the WBC and platelet contents.Methods From June to December 2021,400 mL blood samples were collected from blood donors who met the literature requirements in Quanzhou Blood Center.The blood bags were numbered in the order of 1,2,and 3.The platelet concentrates produced by odd numbered blood were removed WBC by white membrane separation and platelet mixed filtration method,while the platelets concentrates produced by even numbered blood were removed WBC by platelet mixed filtration method.The WBC residual number and platelet count(PLT)content of the two methods were calculated,and the percent of pass of residual WBC and PLT were calculated and compared.Results White membrane filtration was used in 40 bags to remove white blood cellsand40 bags of platelet filtration.The remaining WBC mean values were(4.01±1.33)×10^(5)and(1.63±0.91)×10^(5).The WBC residual values of the former were more than twice that of the latter,and the difference was statistically significant(t=9.34,P<0.05).The mean platinum contents of the two methods were(3.47±1.49)×10^(11)and(2.94±1.03)×10^(11).The PLT content of white membrane separation and platelet mixture filtration method was higher,but the difference was not statistically significant(t=1.85,P>0.05).The qualified rates of residual WBC after leukocyte removal by the two filtration methods were 80.0%and 97.5%,respectively.The qualified rates of residual WBC by platelet mixture filtration method were 97.5%,which was significantly higher,and the difference was statistically significant(P<0.05).The qualified rates of PLT content by the two methods were 85.0%and 75.0%,respectively.But the difference was not statistically significant(P>0.05).Conclusion The platelet mixed filtration method is a good balance to remove WBC and retain platelets,so it is suggested to remove mixed concentrated platelet WBC by platelet mixed filtrat
