自杀相关情绪词量表应用于健康体检的编制和信效度研究

摘　　要：目的:编制自杀相关情绪词量表并检验其信效度,为在健康体检项目中评估心理危机状况提供参考依据。方法:由10名心理危机干预专业人员提名自杀相关情绪词,据此编制相关情绪词量表。采用内部一致性信度,结构效度,项目区分度对量表的信效度进行检验。比较精神心理专业组和非精神心理专业组在判断情绪词与自杀行为关联强度上是否存在差异,并对情绪词与自杀相关行为的关联程度进行探索。结果:量表共收集201名精神心理专业和603名非精神心理专业人员结果。最终量表包括1个因子,共11个条目。结果显示量表的Cronbach'sα系数为0.92。探索性因子分析结果提示,无论是精神心理专业组还是非精神心理专业组评定,均提取一个公因子,解释56.4%的方差变异。项目区分度检验11个条目的临界比值均有统计学意义(P<0.001),11个条目得分与量表总分均显著相关(r=0.58~0.82,P<0.001)。无助、绝望与自杀行为关联强度的组间差异有统计学意义,精神心理专业组判断的关联强度高于非精神心理专业组(t=4.24,4.90,P均<0.001)。重复测量方差分析显示,11个情绪词与自杀行为的关联强度差异有统计学意义(F=332.52,P<0.001),其中绝望与自杀关联最强,以下依次为痛苦、悲伤等。结论:本研究编制的自杀相关情绪词量表信效度良好,在健康体检项目中可应用相应情绪词进行自杀风险筛查,以期进行及时有效干预。Objective:To develop a Suicide-Related Emotion Word Scale and test its reliability and validity.The scale could be used to assess emotional status in mental health screening programs focused on suicide prevention.Methods:Ten suicide prevention professionals nominated candidate emotional words,then classified these words into several categories though consensus discussion.The reliabilities and validities of the scale were tested using internal consistency reliability,construct validity,and item discrimination.The strength scores of association of emotion words suicidal behaviors were compared between mental health professionals and non-mental health professionals,and between 11 emotion words.Results:Two hundred and one mental health professionals and 603 non-mental health professionals were included to test the reliabilities and validities of the scale.The final scale includes 11 items which belong to one factor.The Cronbach’sαcoefficient of the scale was 0.92.The results of the exploratory factor analysis suggested that one main factor could be extracted,for participants of both of mental health professionals and non-mental health professionals.The one-factor model explains 56.4%of the variance.Results of item analysis showed that critical ratios of all 11 items reached statistical significance(P<0.001),and score of each item was significantly correlated with the total score(r=0.58-0.82,P<0.001).Compared with non-mental health professionals,the mental health professionals scored higher associations with suicidal behaviors for helplessness and despair(t=4.24,4.90,both P values<0.001).Results of repeated measures ANOVA showed that the strengths of association with suicidal behavior were statistically significantly different among the 11 emotional words(F=332.52,P<0.001),with despair being the most strongly associated with suicidal behavior,followed by pain and sadness subsequently.Conclusion:The reliabilities and validities of the Suicide-Related Emotional Words Scale developed in this study are acceptable,the s
