摘 要:目的 实验以肩关节轴向拔伸法为干预手段,制作兔肩周炎模型,观察其炎症因子的表达,探讨该手法对肩周炎的疗效机制。方法 把40只实验家兔随机分为4组,每组10只,编入空白组、模型组、对照组、实验组。兔肩周炎模型采用持续劳损加模拟风寒湿邪的方法制作。空白组常规饲养不予造模,也不予干预;模型组常规喂养,造模但不予手法干预;对照组给予肩周炎推拿常规手法;实验组给予肩关节轴向拔伸法,干预20 d后分别取材,取兔耳缘静脉血,检测血清TNF-α、IL-6含量;取模型兔肩周炎局部软组织匀浆,检测PGE-2、TGF-β1含量。结果 肩关节轴向拔伸法通过干预兔肩周炎模型炎症因子表达,从而明显降低肩周炎模型兔静脉血清TNF-α、IL-6含量(P<0.01),同时也可降低局部滑膜及肌肉中的PGE-2、TGF-β1含量,起到较好的抑制炎症作用(P<0.01)。结论 肩关节轴向拔伸法对兔肩周炎模型耳缘静脉血清和患肩局部软组织中的炎症因子有抑制作用,说明可改善局部炎症反应,促进组织修复。Objective: To observe the expression of inflammatory factors and explore the therapeutic mechanism of axial pulling-stretching method on periarthritis of shoulder through the experiment in which the manipulation was used as the intervention means to make rabbit model of periarthritis of shoulder, Methods: 40 experimental rabbits were randomly divided into 4 groups, 10 rabbits each group. They were divided into blank group, model group, control group and experimental group. The rabbit model of periarthritis of shoulder was made by the means of continuous strain and simulating wind cold and dampness. The blank group was conventionally fed without modeling or intervention. The model group was conventionally fed and made models without manual intervention. The control group was given conventional massage for periarthritis of shoulder. The experimental group was given the axial pulling-stretching method of shoulder joint. After 20-day treatment, the rabbit ear margin venous blood was collected, and their serum TNF-α and IL-6 contents were detected. The local soft tissue homogenate of the rabbit model with periarthritis of shoulder were collected and the contents of PGE-2 and TGF-β1 were detected. Results: By intervening the expression of inflammatory factors in the rabbit model of periarthritis of shoulder, the axial pulling-stretching of shoulder joint significantly reduced the contents of TNF-α and IL-6 in the serum of vein of the rabbit model of periarthritis of shoulder(P<0.01), as well as the contents of PGE-2 and TGF-β1 in the local synovium and muscle(P<0.01). Conclusion: The axial pulling-stretching method of shoulder joint has an inhibitory effect on the ear edge venous serum and the inflammatory factors in the local soft tissue of the affected shoulder in the rabbit model of periarthritis of shoulder, which indicates this method can improve the local inflammatory response and promote tissue repair.
