肉芽肿性乳腺炎术后应用阳和汤加减及拔毒生肌散临床观察

摘　　要：目的:观察肉芽肿性乳腺炎(GM)术后应用阳和汤加减及拔毒生肌散的效果。方法:78例分为联合组和外敷组各39例,两组均予以拔毒生肌散外敷,联合组加用阳和汤加减口服。结果:创面愈合率联合组高于外敷组(P<0.05)。治疗后两组创面肉芽评分均提高、VAS评分均下降(P<0.05),且联合组创面肉芽评分高于外敷组、VAS评分低于外敷组(P<0.05)。治疗后两组血清免疫球蛋白(IgA、IgG、IgM)、血管内皮生长因子(VEGF)、碱性成纤维细胞生长因子(bFGF)、转化生长因子β1(TGF-β1)、Ⅲ型胶原蛋白(COLⅢ)水平均上升(P<0.05),且联合组高于外敷组(P<0.05)。治疗后3个月联合组乳房肤色、乳头横向移位、乳头纵向移位、乳房凹陷程度、乳房质地与弹性评分均低于外敷组(P<0.05)。结论:GM术后应用阳和汤及拔毒生肌散效果较好。Objective:To observe the effect of modified Yanghe Decoction and Badu Shengji Powder on granulomatous mastitis(GM)after operation.Methods:Seventy-eight cases were divided into the combined group and the external application group,39 cases in each group.Both groups were treated with external application of Badu Shengji Powder,and the combined group were treated with modified Yanghe Decoction orally in addition.Results:The wound healing rate of the combined group was higher than that of the external application group(P<0.05).After treatment,the wound granulation scores of the two groups increased and the VAS scores decreased(P<0.05),and the wound granulation score of the combined group was higher than that of the external application group,and the VAS score was lower than that of the external application group(P<0.05).After treatment,the levels of serum immunoglobulin(IgA,IgG,IgM),vascular endothelial growth factor(VEGF),basic fibroblast growth factor(bFGF),transforming growth factorβ1(TGF-β1)and type III collagen(COLIII)in the two groups increased(P<0.05),and those in the combined group were higher than those of the external application group(P<0.05).Three months after treatment,the scores of breast skin color,transverse displacement of nipple,longitudinal displacement of nipple,degree of breast depression,breast texture and elasticity in the combined group were lower than those in the external application group(P<0.05).Conclusion:The application of modified Yanghe Decoction and Badu Shengji Powder after GM is effective.
