基于扎根理论的天津市拼凑型紧急医学救援队伍应急能力状况研究

摘　　要：目的:应用扎根理论方法,探讨天津市拼凑型紧急医学救援队伍应急能力状况及其存在的问题,为国家紧急医学救援体系建设提供参考。方法:于2022年3-5月采用目的抽样方法选取天津市15位紧急医学救援队队长进行半结构化深度访谈,并采用扎根理论的方法对数据进行分析。结果:经过三级编码梳理出149个初始概念、25个副范畴、14个主范畴、6个类别及2个核心范畴,并将其归纳为管理结构、内部稳定性和支持力度3个层面,形成拼凑型紧急医学救援队伍存在的弊端与发展对策的研究框架。结论:天津市拼凑型紧急医学救援队伍存在管理结构混乱、内部稳定性弱和支持力度弱的弊端,需要通过标准化的人员准入机制、专职化培训演练机制、多样化激励机制及多主体协同配合机制等来实现队伍管理科学化、稳定性增强和支持力度增加的目标,从而更好发挥紧急医学救援作用。Objective:To explore the status of emergency capability and its existing problems of bricolage emergency medical rescue team in Tianjin based on grounded theory,so as to provide references for the construction of national emergency medical rescue system.Methods:From March to May 2022,15 emergency medical rescue team leaders in Tianjin were selected by the purposive sampling method to conduct semi-structured in-depth interviews,and the grounded theory method was used to analyze the data.Results:After three-rank coding,149 initial concepts,25 sub-categories,14 main categories,6 categories and 2 core categories were sorted out.They were categorized into 3 levels including management structure,internal stability and support.At the last,the research framework of the disadvantages and development countermeasures of the bricolage emergency medical rescue team was formed.Conclusion:The bricolage emergency medical rescue teams in Tianjin exist the disadvantages of chaotic management structure,weak internal stability and weak team support.It is necessary to achieve the goals of scientific team management,enhanced stability and increased support through standardized personnel access mechanism,full-time training and exercise mechanism,diversified incentive mechanism and multi-agent coordination mechanism,so as to better play the role of emergency medical rescue.
