摘 要:目的探讨职业性尘肺病患者血清癌胚抗原(CEA)、糖类抗原125(CA125)表达水平和体液免疫功能情况及其对于临床实践的指导意义。方法选取2019年1月-2020年6月期间我院收治的123例男性尘肺病患者作为尘肺组,另选同期体检肺部健康男性56例作为对照组。检测并对比两组人员的肺纤维化水平、血清CEA、CA125和体液免疫水平等指标,同时分析血清CEA、CA125与尘肺病患者体液免疫水平及肺纤维化之间的关系。结果尘肺病组肺纤维化指标水平均高于对照组(t_(1)=50.949,t_(2)=25.656,t_(3)=10.900,P<0.05)。尘肺病组体液免疫指标、CEA和CA125水平均高于对照组(t_(1)=5.765,t_(2)=2.996,t_(3)=7.002,t_(4)=31.396,t_(5)=20.537,P<0.05)。煤工尘肺患者体液免疫指标、CEA和CA125水平均高于矽肺患者(t_(1)=2.381,t_(2)=3.522,t_(3)=4.204,t_(4)=6.992,t_(5)=11.792,P<0.05)。叁期矽肺和煤工尘肺患者体液免疫指标、CEA和CA125水平均高于壹期同病种患者(F_(1)=3.788,F_(2)=3.963,F_(3)=4.823,F_(4)=3.411,F_(5)=5.077,F_(6)=4.192,F_(7)=6.602,F_(8)=3.189,F_(9)=4.436,F_(10)=4.429,P<0.05)。比较发现不同接尘年限的矽肺和煤工尘肺患者的体液免疫指标、CEA和CA125水平之间无明显差异(F_(1)=0.002,F_(2)=0.006,F_(3)=0.142,F_(4)=0.022,F_(5)=0.215,F_(6)=0.005,F_(7)=0.037,F_(8)=0.321,F_(9)=0.030,F_(10)=0.080,P>0.05)。通过Pearson分析发现以上体液免疫指标、CEA、CA125与肺纤维化指标均呈正相关性(r_(1)=0.634,r_(2)=0.624,r_(3)=0.628,r_(4)=0.618,r_(5)=0.612,r_(6)=0.594,r_(7)=0.621,r_(8)=0.596,r_(9)=0.631,r_(10)=0.648,r_(11)=0.653,r_(12)=0.667,r_(13)=0.658,r_(14)=0.674,r_(15)=0.664,P<0.05)。结论职业性尘肺病患者体内体液免疫功能明显增强,且血清中CEA和CA125高表达,与疾病发生和发展有关。Objective To detect serum carcinoembryonic antigen(CEA)and carbohydrate antigen 125(CA125)levels and examine the humoral immunity function in patients with occupational pneumoconiosis,and to discuss their clinical significance.Methods A sample of 123 male patients with occupational pneumoconiosis and another 56 healthy men in our hospital from January 2019 to June 2020 were enrolled as pneumoconiosis group and healthy control group,respectively.The pulmonary fibrosis indexes,serum CEA,CA125 levels and humoral immunity parameters were compared between groups.Then the correlation of serum CEA and CA125 levels withhumoral immunity and pulmonary fibrosis parameterswas analyzed.Results The levels ofpulmonary fibrosis indexes were significantly higher in pneumo⁃coniosis group than in control group(t_(1)=50.949,t_(2)=25.656,t_(3)=10.900,all P<0.05).The levels of humoral immunity parameters,serum CEA and CA125 of pneumoconiosis group were significantly higher compared with those of control group(t_(1)=5.765,t_(2)=2.996,t_(3)=7.002,t_(4)=31.396,t_(5)=20.537,all P<0.05).The levels of humoral immunity parameters,serum CEA and CA125 of coal workers'pneumoconiosis patients were significantly higher than those of silicosis patients(t_(1)=2.381,t_(2)=3.522,t_(3)=4.204,t_(4)=6.992,t_(5)=11.792,all P<0.05).The levels of humoral immunity parameters,serum CEA and CA125 of silicosis patients and coal workers'pneumoconiosis patientsat stage III were significantly higher than those of patients at at stage I,with statistical difference(F_(1)=3.788,F_(2)=3.963,F_(3)=4.823,F_(4)=3.411,F_(5)=5.077,F_(6)=4.192,F_(7)=6.602,F_(8)=3.189,F_(9)=4.436,F_(10)=4.429,all P<0.05).The levels of humoral immunity parameters,CEA and CA125 of silicosis and coal workers'pneumoconiosis patients showed no significant difference in terms of the dust exposure durations(F_(1)=0.002,F_(2)=0.006,F_(3)=0.142,F_(4)=0.022,F_(5)=0.215,F_(6)=0.005,F_(7)=0.037,F_(8)=0.321,F_(9)=0.030,F_(10)=0.080,all P>0.05).Pearson correlation analysis showed that humoral immunity
