摘 要:目的探讨机器人援助腹腔镜操作在复杂性子宫切除术中的可行性及优势。方法回顾性分析经机器人腹腔镜下复杂性子宫切除术的36例患者以及同期进行传统腹腔镜下复杂性子宫切除术的85例患者的临床资料,包括患者手术时长、术中失血量、首次肛门排气时间、术后住院天数、术后12 h疼痛视觉模拟量表(VAS)评分、是否中转开腹、手术并发症(肠梗阻、切口疝、切口感染、双下肢深静脉血栓、泌尿系损伤、阴道断端愈合不良)。结果机器人组患者的子宫体积、术中失血量、首次肛门排气时间、术后住院天数、术后12 h VAS评分、中转开腹率均低于腹腔镜组,手术时间略长于腹腔镜组,差异有统计学意义(P<0.05)。2组患者术后并发症(肠梗阻、切口疝、切口感染、双下肢深静脉血栓、泌尿系损伤、阴道断端愈合不良)发生率比较,差异无统计学意义(P>0.05)。结论机器人腹腔镜手术定位精准,操作灵活,能明显减轻术后不适,减少术中出血量,促进患者恢复,适合复杂性子宫切除术。Objective To discuss the feasibility and advantages of robot-assisted laparoscopic in complex hysterectomy.Methods The clinical data of 36 patients undergoing complex hysterectomy under robotic laparoscopy and 85 patients undergoing complex hysterectomy under traditional laparoscopy at the same time were retrospectively analyzed.The length of operation,blood loss during operation,first exhaust time,length of postoperative hospital stay,visual analogue scale(VAS)score at postoperative 12 h,transit abdominal opening rate,surgical complications(intestinal obstruction,incisional hernia,incisional infection,deep vein thrombosis of both lower extremities,urinary tract injury,and poor healing of the broken vagina)were analyzed.Results The size of the uterus,intraoperative blood loss,first exhaust time,postoperative hospital stay days,VAS score at postoperative 12 h,and transit abdominal opening rate of the robot group were all lower than those in the laparoscopic group,and the operation time was slightly longer than those in the laparoscopic group(P<0.05).There was no significant difference in the incidence of postoperative complications between the two groups(P>0.05).Conclusion With accurate positioning and flexible operation,robotic laparoscopic surgery can reduce postoperative discomfort,reduce intraoperative blood loss and promote patient recovery,and make it more suitable for complicated hysterectomy.
