心电图碎裂QRS波、QT离散度评估急性心肌梗死患者PCI术后恶性心律失常发生的价值

摘　　要：目的分析心电图碎裂QRS波(fQRS)、QT离散度(QTd)评估急性心肌梗死(AMI)患者经皮冠状动脉介入治疗(PCI)术后恶性心律失常(MA)发生的价值。方法抽取2020年1月至2021年3月于滨州市中心医院行PCI的100例AMI患者为研究对象,术后随访72 h,根据患者MA发生情况分为MA组与非MA组。调查并记录患者的相关资料,检查患者入院时心电图fQRS、QTd值,分析心电图fQRS、QTd对AMI患者PCI后MA发生的评估价值。结果术后72 h内,100例患者中27例(27.00%)发生MA;MA组患者心电图存在fQRS者占比(66.67%,21/27)高于非MA组患者(36.99%,15/73),心电图QTd值[(51.86±3.35)ms]大于非MA组患者[(44.23±3.49)ms],P<0.05。心电图fQRS、QTd评估AMI患者PCI后MA发生的受试者工作特征曲线下面积分别为0.786、0.813。结论心电图fQRS、QTd对AMI患者PCI术后MA发生具有一定评估价值。Objective To analyze the value of electrocardiogram(ECG)fragmented QRS wave(fQRS)and QT dispersion(QTd)in predicting the occurrence of malignant arrhythmia(MA)after percutaneous coronary intervention(PCI)in patients with acute myocardial infarction(AMI).Methods A total of 100 AMI patients who underwent PCI in Binzhou Central Hospital from January 2020 to March 2021 were selected as the research objects.They were followed up for 72 hours after operation.The patients were divided into MA group and non MA group according to the occurrence of MA.The relevant data of patients were investigated and recorded,the ECG fQRS and QTd values at admission were checked,and the predictive value of ECG fQRS and QTd on MA after PCI in patients with AMI was analyzed.Results Within 72 hours after operation,MA occurred in 27 of 100 patients,accounting for 27.00%.The proportion of fQRS of ECG in the MA group(66.67%,21/27)was higher than that in the non MA group(36.99%,15/73),and the ECG QTd value was(51.86±3.35)ms,higher than the(44.23±3.49)ms in the non MA group(P<0.05).The area under the curve of operating characteristic curve of MA after PCI in AMI patients assessed by ECG fQRS and QTd were 0.786 and 0.813,respectively.Conclusions fQRS and QTd in ECG have certain value in predicting the occurrence of MA after PCI in patients with AMI.
