摘 要:目的探讨高频超声在类风湿关节炎患者指关节病变诊断中的应用价值。方法抽取2018年10月至2020年10月武警河南总队医院收治的类风湿关节炎患者85例作为研究组,另抽取同期行健康体检的85例健康人作为对照组。两组均使用高频超声诊断患者的指关节病变。比较两组骨侵蚀、血管翳、关节积液、滑膜增厚、滑膜增厚、肌腱腱鞘炎检出情况,各手指关节滑膜厚度测量结果。结果研究组骨侵蚀、血管翳、关节积液、滑膜增厚、肌腱腱鞘炎检出率高于对照组(P<0.05);研究组左手第5掌指关节、左手第4指间关节、左手第3指间关节、左手第3掌指关节、左手第2掌指关节、左手第1指间关节、右手第3掌指关节、右手第2指间关节、右手第2掌指关节、右手第1指间关节、右手第1掌指关节滑膜厚度均高于对照组(P均<0.05)。结论在类风湿关节炎患者指关节病变的诊断中,高频超声可以充分了解患者的病理改变。Objective To investigate the application value of high frequency ultrasound in the diagnosis of finger joint lesions in patients with rheumatoid arthritis.Methods Eighty-five patients with rheumatoid arthritis treated in Henan Armed Police Corps Hospital from October 2018 to October 2020 were selected as the study group,and 85 healthy people who underwent physical examination in the hospital at the same time were selected as the control group.High frequency ultrasound was used to diagnose finger joint.The detection of bone erosion,pannus,joint effusion,synovial thickening,synovial thickening,tenosynovitis and synovial thickness of each finger joint were compared between the two groups.Results The detection rate of bone erosion,pannus,joint effusion,synovial thickening and tenosynovitis in the study group were higher than those in the control group(P<0.05).The synovial thickness of the fifth metacarpophalangeal joint of the left hand,the fourth interphalangeal joint of the left hand,the third interphalangeal joint of the left hand,the third metacarpophalangeal joint of the left hand,the second metacarpophalangeal joint of the left hand,the first interphalangeal joint of the left hand,the third metacarpophalangeal joint of the right hand,the second interphalangeal joint of the right hand,the second metacarpophalangeal joint of the right hand,the first interphalangeal joint of the right hand,and the first metacarpophalangeal joint of the right hand in the study group were all higher than those in the control group(all P<0.05).Conclusions In the diagnosis of finger joint lesions in patients with rheumatoid arthritis,high-frequency ultrasound can fully display the pathological changes of patients.
