刘丽芳教授基于“玄府气液”学说运用坐浴法治疗肛门瘙痒症

摘　　要：介绍湖南省名中医刘丽芳对肛门瘙痒症的临证经验。刘丽芳首次从“玄府气液”微观辨证角度出发,阐释肛门瘙痒症的发病机制及坐浴法的治疗思路。认为该病病位核心在玄府,治疗根本在于“宣通玄府,调和气液”。急性肛门瘙痒症以实证为主,病机以“玄府不固,外感六淫”为基础,治则以“宣通玄府”为主。慢性肛门瘙痒症以正虚为本,“玄府虚损,气液失调”是其发生、发展以及日久不愈的内在关键,治则以“调和气液”为根本。坐浴法作为中医外治特色疗法,可宣通玄府、调和气液,使玄府开阖有度,气液通达,邪退病愈。结合临床验案,坐浴法可作为治疗肛门瘙痒症的主要外治手段。This paper has summarized Professor LIU Lifang’s clinical experience of treating pruritus ani(PA). As a famous Chinese medicine doctor in Hunan, Professor LIU for the first time has explained the pathogenesis and therapeutic thought of PA from microscopic syndrome differentiation of "qi and fluid through mysterious mansion". It is believed that the location of the disease mainly is the mystrious mansion, so the treatment should be focused on "opening the mysterious mansion, regulating qi and fluids". The acute PA is predominantly manifested as the excess pattern, and its pathogenesis is based on "unstable the mysterious mansion and externally-contracted six pathogenic factors", thus the main treatment principle is to "open the mysterious mansion".The chronic PA is mainly manifested as the deficiency pattern, and the deficiency of the mysterious mansion and the imbalance of qi and fluids are the internal key to its occurrence, development and prolonged unhealing, so the main treatment principle is "regulating qi and fluids". As a characteristic external treatment of Chinese medicine, the hip bath therapy can open the mysterious mansion and regulate qi and fluids, thus make the the mysterious mansion open properly, qi and fluids move smoothly, then the disease can be cured. This paper has expounded that hip bath therapy could be the main external treatment for PA based on Professor LIU’s clinical experience.
