珠海市大学生首次性行为安全套使用与最近一年性行为的关系分析

摘　　要：目的分析首次性行为安全套使用对最近一年性行为的发生和安全套使用的影响,为大学生开展有针对性的艾滋病防控措施和性健康教育提供参考依据。方法采用多阶段整群抽样方法,对珠海市6所高校内的学生进行匿名调查,收集首次性行为安全套使用情况及最近一年性行为情况等信息。结果共纳入1789名有性经历的学生,首次性行为安全套使用率为73.9%(1322/1789),最近一年安全套持续使用率为56.4%(807/1430),最近一次性行为安全套使用率为81.8%(1170/1430)。多因素分析显示,首次性行为使用安全套的学生在最近一年发生性行为(a OR=1.65,95%CI:1.27~2.14)、固定性伴性行为(a OR=1.39,95%CI:1.01~1.91)的可能性较高,而发生多性伴性行为(a OR=0.68,95%CI:0.51~0.93)的可能性较低。首次性行为使用安全套的学生在最近一年性行为(a OR=6.75,95%CI:5.03~9.07)、最近一年与固定性伴(a OR=5.15,95%CI:3.75~7.07)、临时性伴(a OR=5.06,95%CI:2.82~9.07)、商业性性伴(a OR=4.90,95%CI:1.53~15.64)性行为时更倾向持续使用安全套;在最近一次性行为(a OR=3.78,95%CI:3.00~4.76)、最近一次与固定性伴(a OR=4.62,95%CI:3.13~6.84)、临时性伴(a OR=3.57,95%CI:1.72~7.40)性行为时更倾向使用安全套。结论珠海市在校大学生首次性行为安全套使用率较高,首次性行为使用安全套可以促进最近一年性行为使用安全套,学校需及早进行全面的性教育,帮助学生养成安全性行为的习惯。Objective To explore the association between condom use at first intercourse and sexual behavior in the past year among college students,aiming to provide a reference for targeted AIDS prevention and control measures and sexual health education programs among the students.Methods We used the multistage sampling method to recruit college students from 6 colleges and universities in Zhuhai.An anonymous questionnaire was distributed to collect data on condom use at first intercourse and sexual behavior in the past year.Results Among 1789 students,73.9%used condoms at first intercourse,56.4%used condoms consistently in the past year and 81.8%used condoms at the last intercourse.Multivariable analysis showed that college students who used condoms at first intercourse were more likely to have sexual intercourse(a OR=1.65,95%CI:1.27-2.14)and to have sex with regular sexual partners(a OR=1.39,95%CI:1.01-1.91)in the past one year.However,they had a lower risk of having sex with multiple sexual partners(a OR=0.68,95%CI:0.51-0.93)in the past year.Moreover,condom use at first intercourse was also associated with consistent condom use in the most recent sexual intercourse(a OR=6.75,95%CI:5.03-9.07),the most recent intercourse with regular sexual partners(a OR=5.15,95%CI:3.75-7.07),when having causal sexual partners(a OR=5.06,95%CI:2.82-9.07),and when having commercial sexual partners(a OR=4.90,95%CI:1.53-15.64).Students who used condoms at first intercourse were more likely to use condoms in the most recent sexual intercourse(a OR=3.78,95%CI:3.00-4.76),when having sex with regular sexual partners(a OR=4.62,95%CI:3.13-6.84)and when causal sexual partners(a OR=3.57,95%CI:1.72-7.40).Conclusions The rate of condom use at first intercourse among college students in Zhuhai was high.Condom use at first intercourse can facilitate safe sexual behavior in the past year.Schools need to implement comprehensive sex education as early as possible to help students build the habit of condom use.
