摘 要:目的观察间歇有氧训练对低氧肺动脉高压(hypoxic pulmonary arterial hypertension,HPAH)大鼠的肺动脉压及肺小动脉结构的影响,探讨间歇有氧训练改善HPAH的机制。方法30只雄性SD大鼠随机分为对照组、HPAH组、康复组,每组各10只。HPAH组及康复组给予低氧环境(氧浓度9%~11%,8 h/d,持续4周)建立大鼠HPAH模型。康复组在造模后给予间歇有氧训练4周。干预完成后测定各组大鼠的平均肺动脉压(MPAP)与右心室肥厚指数(RVHI);观察肺动脉病理学改变并测定肺小动脉管壁厚度/血管外径(MT%);检测自噬相关指标Beclin-1、LC3的蛋白表达。结果HPAH大鼠MPAP、RVHI指标与对照组(12.52±2.60)mmHg(1 mm Hg=0.133 k Pa)、(21.37±3.75)%相比,HPAH组和康复组的MPAP、RVHI均升高,但康复组(26.14±4.21)mmHg、(28.09±4.43)%低于HPAH组(40.41±3.97)mmHg、(47.92±5.57)%,差异具有统计学意义(P<0.05)。HPAH组MT%(51.77±4.44)%明显高于对照组(29.38±3.07)%,肺血管重构明显;康复组MT%(40.07±2.95)%较HPAH组明显下降,肺血管重构程度减轻。HPAH组和康复组大鼠肺组织自噬标志性蛋白Beclin-1、LC3蛋白表达较对照组均增加;但康复组Beclin-1、LC3蛋白表达明显高于HPAH组(P<0.05)。结论间歇有氧康复训练可降低肺动脉压,改善HPAH大鼠肺动脉重构,其机制可能与间歇有氧康复训练激活自噬相关。Objective To observe the effect of intermittent aerobic training on pulmonary artery pressure and pulmonary arterial structure in hypoxic pulmonary arterial hypertension(HPAH)rats,and to explore the mechanism of intermittent aerobic training to improve HPAH.Methods Thirty male SD rats were randomly divided into control group,HPAH group and rehabilitation group,with 10 rats in each group.HPAH group and rehabilitation group were given hypoxic environment(oxygen concentration of 9%~11%,8 hours/day,lasting for 4 weeks)to establish rat HPAH model.The rehabilitation group was given intermittent aerobic training for 4 weeks after modeling.After the intervention,the mean pulmonary artery pressure(MPAP)and right ventricular hypertrophy index(RVHI)were measured;The pathological changes of pulmonary arteries were observed and the wall thickness/external diameter(MT%)of pulmonary arterioles were measured;The protein expression of autophagy related Beclin-1 and LC3 were detected.Results Compared with the control group(12.52±2.60)mm Hg(1 mm Hg=0.133 k Pa),(21.37±3.75)%,the MPAP and RVHI indexes of HPAH rats in the HPAH group and the rehabilitation group increased,but those in the rehabilitation group(26.14±4.21)mm Hg,(28.09±4.43)%were lower than those in the HPAH group(40.41±3.97)mmHg and(47.92±5.57)%.The difference was statistically significant(P<0.05).MT%(51.77±4.44)%in the HPAH group was significantly higher than that in the control group(29.38±3.07)%,and pulmonary duct remodeling was obvious;MT%(40.07±2.95)%in the rehabilitation group was significantly lower than that in the HPAH group,and the degree of pulmonary vascular remodeling was reduced.The expression of Beclin-1 and LC3 protein in lung tissue of rats in the HPAH group and the rehabilitation group was higher than that in the control group;However,the expression of Beclin-1 and LC3 protein in the rehabilitation group was significantly higher than that in the HPAH group(P<0.05).Conclusion Intermittent aerobic rehabilitation training can reduce pulmonary arte
