黄芪中药制剂治疗骨质疏松症疗效及安全性的Meta分析

摘　　要：目的系统评价含黄芪中药制剂治疗骨质疏松症的疗效。方法利用计算机检索各大数据库,如中国知网(CNKI)、中国生物医学文献服务系统、维普(VIP)、万方(Wanfang)、Pub Med、Cochrane图书馆、EMbase、Web of science等数据库,检索时间均以各数据库建库时间至2022年9月含黄芪中药制剂治疗骨质疏松的随机对照试验(randomized controlled trial,RCT),并采用Rev Man 5.4软件对符合纳入条件的文献进行Meta分析。结果共纳入19篇RCT文献,研究共涉及1373例患者。Meta分析结果显示:与对照组相比,含黄芪中药制剂作为观察组治疗骨质疏松症的临床疗效[OR=4.01,95%CI(2.77,5.81),P<0.00001],骨密度[MD=0.10,95%CI(0.05,0.15),P=0.0002],中医证候评分[MD=-2.51,95%CI(-3.70,-1.33),P<0.0001],疼痛评分[MD=-0.87,95%CI(-1.33,-0.41),P=0.0002],差异均有统计学意义。结论与单用西药或中成药相比,西药或中成药联合含黄芪中药制剂可有效提升骨质疏松患者临床疗效、改善临床症状、增加骨密度,缓解疼痛。Objective To systematically evaluate the therapeutic effect of traditional Chinese medicine containing astragalus membranaceus on osteoporosis.Methods All major databases,such as CNKI,Cochrane Library,Wanfang,PubMed,VIP,and Web of Science,were retrieved by computer.Randomized controlled trials(RCT)of traditional Chinese medicine preparations containing astragalus membranaceus in the treatment of osteoporosis till September 2022was retrieved,and the literatures eligible for inclusion were Meta-analysed with RevMan 5.4 software.Results A total of 19 RCT literatures were included,involving 1373 patients.Meta-analysis results showed that:compared with the control group,there were significant differences in the clinical efficacy[OR=4.01,95%CI(2.77,5.81),P<0.00001],bone mineral density[MD=0.10,95%CI(0.05,0.15),P=0.0002],TCM syndrome score[MD=-2.51,95%CI(-3.70,-1.33),P<0.0001],pain score[MD=-0.87,95%CI(-1.33,-0.41),P=0.0002]of the observation group in the treatment of osteoporosis with traditional Chinese medicine containing astragalus membranaceus.Conclusion Compared with western medicine or traditional Chinese medicine alone,western medicine or traditional Chinese medicine combined with traditional Chinese medicine containing astragalus membranaceus can effectively improve the clinical efficacy of osteoporosis patients,improve clinical symptoms,increase bone density and relieve pain.
