“整颈三步九法”联合颈复康颗粒对神经根型颈椎病患者颈椎功能和疼痛因子的影响

摘　　要：目的探讨“整颈三步九法”联合颈复康颗粒对神经根型颈椎病患者颈椎功能和疼痛因子的影响。方法选择2020年1月至2021年12月在同济大学医学院附属静安区彭浦新村街道社区卫生服务中心治疗的110例神经根型颈椎病患者,按照随机数字表分为两组。对照组55例口服颈复康颗粒治疗,联合组55例在此基础上,再给予规范的“整颈三步九法”治疗。比较两组颈椎功能障碍指数(NDI)评分,简化Mc Gill疼痛问卷(SF-MPQ)积分,临床疗效,血清疼痛因子水平及复发情况。结果与治疗前相比,治疗后、治疗后1、3个月两组NDI评分、SF-MPQ积分、P物质(SP)、前列腺素E2(PGE2)、5-羟色胺(5-HT)水平均降低,联合组均低于对照组,差异有统计学意义(P<0.05)。联合组总有效率90.91%高于对照组74.55%,治疗后3个月的复发率5.45%低于对照组18.18%,差异有统计学意义(P<0.05)。结论“整颈三步九法”联合颈复康颗粒治疗神经根型颈椎病效果确切,可提高患者颈椎功能,减轻疼痛,减少血清疼痛因子水平,且复发率较低。Objective To investigate the effect of"three steps and nine methods of cervical adjustment"combined with Jingfukang granules on cervical function and pain factors in patients with cervical spondylotic radiculopathy.Methods From January 2020 to December 2021,110 patients with cervical spondylotic radiculopathy who were treated at the Community Health Service Center of Pengpu Xincun Neighborhood,Jing’an District,affiliated to Tongji University Medical College,were divided into two groups by random number table.55 cases in the control group were treated with Jingfukang granules orally,and 55 cases in the combined group were treated with the standardized"three steps and nine methods of cervical adjustment"on this basis.The scores of neck disability index(NDI),scores of simplified McGill pain questionnaire(SF-MPQ),clinical efficacy,serum pain factor level and recurrence were compared between the two groups.Results Compared with before treatment,NDI scores,SF-MPQ scores,substance P(SP),prostaglandin E2(PGE2),and 5-hydroxytryptamine(5-HT)levels in the two groups decreased after treatment,1 and 3months after treatment,and the combined group was lower than the control group,with a statistically significant difference(P<0.05).The total effective rate of the combined group was 90.91%higher than that of the control group74.55%,and the recurrence rate of 5.45%after 3 months of treatment was lower than that of the control group18.18%,with a statistically significant differenc(P<0.05).Conclusion The"three steps and nine methods of cervical adjustment"combined with Jingfukang granules has a definite effect on the treatment of cervical spondylotic radiculopathy,which can improve the cervical function of patients,relieve pain,reduce the level of serum pain factors,and have a low recurrence rate.
