摘　　要：目的:分析5-氨基酮戊酸光动力疗法(ALA-PDT)联合二氧化碳激光治疗肛管内尖锐湿疣的临床疗效。方法:选取2020年1月-2021年7月苏州市第五人民医院皮肤性病科收治的肛管内尖锐湿疣男性患者10例为观察对象,行ALA-PDT联合二氧化碳激光治疗,观察患者治疗效果。结果:10例患者均经过3~6次ALA-PDT治疗及多次二氧化碳激光,行ALA-PDT治疗≤4次患者5例,>4次患者5例,二氧化碳激光≤4次6例,>4次4例。10例患者治疗时间3~13个月,中位数7个月,末次治疗后>6个月均未复发,5例患者CD4^(+)T细胞计数<500 cells/μL,治疗时间为3~13个月,中位数9个月;10例患者在ALA-PDT治疗期间均出现疼痛反应,在可耐受范围,无需应用镇痛药物。术后部分患者肛门出现轻度水肿,2 d内自行消退,无需特殊处理。结论:ALA-PDT联合二氧化碳激光治疗肛管内尖锐湿疣有效。Objective:To analyze the clinical effect of 5-amino-ketovalerate photodynamic therapy(ALA-PDT)combined with carbon dioxide laser in the treatment of condyloma acuminatum in anal canal.Methods:Ten male patients with condyloma acuminatum in anal canal admitted to the department of dermatology of the Fifth People's Hospital of Suzhou from January 2020 to July 2021 were selected as the observation objects.ALA-PDT combined with carbon dioxide laser therapy was performed to observe the therapeutic effect.Results:All the 10 patients were treated with ALA-PDT for 3~6 times and carbon dioxide laser for multiple times.5 patients received ALA-PDT treatment≤4 times,5 patients>4 times,6 patients≤4 times CO_(2) laser treatment,4 patients>4 times.10 patients were treated for 3 to 13 months,with a median of 7 months,and no recurrence occurred>6 months after the last treatment.Five patients with CD4^(+)T cell count<500 cells/μL were treated for 3 to 13 months with a median of 9 months.All the 10 patients experienced pain reactions during ALA-PDT treatment,which were within the tolerable range and no analgesic drugs were needed.Mild anal edema appeared in some patients after surgery,which subsided spontaneously within 2 days without special treatment.Conclusion:ALA-PDT combined with carbon dioxide laser is effective in treating condyloma acuminatum in anal canal.
