康复医学带教在骨科住院医师规范化培训中的实践及探讨

摘　　要：住院医师规范化培训是医学生毕业后教育的重要组成部分。重庆医科大学附属第一医院骨科专业住院医师规范化培训均需进行为期1月的康复医学科轮转,进行骨科康复相关知识及技能培训。文章结合骨科康复带教实践经验,对骨科住院医师住培进行骨科康复轮转教学进行探讨,提出应重视小讲座学习,着重讲授常见骨折肢体运动功能的康复评定及康复治疗原则,以及颈肩腰腿痛等退行性疾病的非手术治疗适应证及治疗方法;高质量的临床教学培训,尤其是针对颈肩腰腿痛病例、肩峰撞击综合征及肩袖损伤等病例,可将医疗查房与教学查房相结合,通过骨科住院住培医师患者收治及亲自参与各种临床治疗及效果反馈过程,切身体会及感受讲授手术治疗与非手术治疗的各自适应证、各自优势及不足;优化出科考核内容,提高骨科住院医师规范化培训的骨科康复带教质量。The standardized training of resident physicians is an important part of the post-graduate education of medical students.The standardized training for orthopaedic residents in the First Affiliated Hospital of Chongqing Medical University requires a one-month rotation in the rehabilitation medicine department to conduct orthopaedic rehabilitation-related knowledge and skills training.Based on the practical experience of orthopaedic rehabilitation teaching,this article discusses the rotation teaching of orthopaedic rehabilitation for orthopaedic residents,and proposes that attention should be paid to small lectures,focusing on the rehabilitation evaluation and rehabilitation treatment principles of common fractured limbs,cervical and lumbar pathology as well as shoulder and hip disorders,especially on non-surgical treatment indications and treatment methods for degenerative diseases.It is important to conduct high-quality clinical teaching and training,by cases of neck,shoulder,lumbar and leg pain,acromion impingement syndrome and rotator cuff injury,combined clinical physical examination and teaching demonstration.Through participation in a variety of orthopaedic rehabilitation-related medical treatments and effect feedback processes,resident physicians in major of orthopaedic can be familiar with advantages and disadvantages of surgical treatment and non-surgical treatment,and the quality of orthopaedic rehabilitation teaching in the standardized training of orthopaedic residents is improved.
