摘 要:目的:研究胎儿颈项透明层(NT)厚度超声检测联合母体外周血单核细胞(PBMCs)受体相互作用蛋白激酶1(RIPK1)、微小核糖核酸-22-3p(miR-22-3p)在胎儿先天性心脏病(CHD)筛查中的价值并予以分析。方法:回顾性分析2019年2月-2021年2月于佳木斯市中心医院接受规律性产检,且随访明确胎儿伴有CHD的41例孕妇的临床资料,记作研究组。另取同期于医院接受规律性产检,且随访明确胎儿无CHD的41例孕妇作为对照组。均进行NT厚度超声检测及母体PBMCs RIPK1、miR-22-3p表达水平检测。以随访结局作为金标准,分析NT厚度及母体PBMCs RIPK1、miR-22-3p表达水平对胎儿CHD的诊断效能。结果:研究组NT厚度及PBMCs RIPK1水平分别为(2.80±0.34)mm、(1.44±0.46),均高于对照组的(2.15±0.23)mm、(1.02±0.36),而miR-22-3p水平为(0.79±0.21),低于对照组的(1.22±0.25)(P<0.05)。以随访结局作为金标准,NT厚度及PBMCs RIPK1、miR-22-3p表达水平联合诊断胎儿CHD的敏感度、特异度以及准确度分别为97.56%、97.56%、97.56%,均高于NT厚度单独诊断的75.61%、82.93%、79.27%,PBMCs RIPK1单独诊断的78.05%、80.49%、79.27%,PBMCs miR-22-3p单独诊断的73.17%、80.49%、76.83%(P<0.05)。结论:孕早期检测NT厚度及母体PBMCs RIPK1、miR-22-3p表达水平可为胎儿CHD的筛查提供参考依据。Objective:To investigate and analyze the value of ultrasound detection of fetal neck transparent(NT)layer thickness combined with maternal peripheral blood mononuclear cells(PBMCs)receptor interacting protein kinase 1(RIPK1)and microRNAs 22-3p(miR-22-3p)in screening fetal congenital heart disease(CHD).Method:The clinical data of 41 pregnant women who received regular birth examination in Jiamusi Central Hospital from February 2019 to February 2021 and were followed up with clear fetal CHD were analyzed retrospectively and recorded as the study group,another 41 pregnant women who received regular birth examination in hospital during the same period and were followed up to confirm that the fetal had no CHD were selected as the control group.All subjects were tested for NT thickness ultrasonography and expression levels of maternal PBMCs RIPK1 and miR-22-3p.Follow-up outcome was used as the gold standard to analyze the diagnostic efficacy of NT thickness and maternal PBMCs RIPK1 and miR-22-3p expression levels for fetal CHD.Result:NT thickness and PBMCs RIPK1 level in the study group were(2.80±0.34)mm and(1.44±0.46)respectively,which were higher than(2.15±0.23)mm and(1.02±0.36)in the control group,while miR-22-3p level in the study group was(0.79±0.21),which was lower than(1.22±0.25)in the control group(P<0.05).Using follow-up outcome as the gold standard,the sensitivity,specificity and accuracy of NT thickness combined with PBMCs RIPK1 and miR-22-3p expression levels in the diagnosis of fetal CHD were 97.56%,97.56%and 97.56%,respectively,which were higher than 75.61%,82.93%,79.27%of NT thickness alone,78.05%,80.49%,79.27%of PBMCs RIPK1 alone,and 73.17%,80.49%,76.83%of PBMCs miR-22-3p were diagnosed separately(P<0.05).Conclusion:Detection of NT thickness and expression levels of maternal PBMCs RIPK1 and miR-22-3p in early pregnancy can provide reference for screening fetal CHD.
