摘  要:目的 通过观察高仿真呼吸系统纤维支气管镜训练模拟器在纤维支气管镜教学中的效果,探讨高仿真模拟教学在纤维支气管镜临床教学中的作用。方法 选取2020年11月—2021年10月在麻醉科实习或轮转的培训医师或实习医师40人,根据教学方法不同随机分为两组,每组20人,对照组采用传统的理论授课和临床观摩的方法进行培训,观察组则采用理论授课、临床观摩、训练模拟器模拟训练等方法进行培训。两组学员在年龄、性别、工作年限等一般情况上比较差异无统计学意义(P>0.05)。分别对两组学员培训前后进行理论考核和模拟操作考核,记录两组学员理论考核成绩、在模拟器上完成纤维支气管镜检查所需时间以及临床实践中3 min内引导插管成功率。记录两组学员培训前后理论考试成绩的提高值以及模拟器上模拟操作所用时间的缩短值。结果 与对照组比较,观察组培训后理论考核提高成绩为(23±8)分,高于对照组提高的成绩(11±7)分,差异有统计学意义(P<0.05)。培训前后对照组模拟器上完成纤维支气管镜检查所需时间缩短(51±31)s,明显少于观察组缩短时间(96±26)s,差异有统计学意义(P<0.05)。与对照组比较,临床实践中,观察组3 min内纤维支气管镜引导插管成功率显著高于对照组,差异有统计学意义(P<0.05)。结论 高仿真模拟教学能提高学员理论成绩和实践考核成绩,能够提高纤维支气管镜教学的效果和临床实践中插管成功率。Objective To study the effect of high simulation training simulator on bronchoscopy teaching and learning.Methods A total of 40 trainee physicians or interns who practiced or rotated in the Department of Anesthesiology from November, 2020 to October, 2021 were engaged in the study. They were randomly assigned to a control group and an observation group with 20 in each group. In the control group, traditional theoretical teaching and clinical practice were adopted for the training and in the observation group the students were trained on theoretical teaching, clinical observation and simulation training with a simulator. There was no statistical difference between the two groups in terms of age, gender, working years, and other demographic data. The theoretical assessment and simulated operation assessment were conducted among the trainees of two groups before and after the training, with the theoretical assessment results, the time required to complete fiber-optic bronchoscopy on the simulator, and the success rate of guided intubation within 3 minutes in clinical practice recorded. The improvement value of the theoretical examination scores of the two groups of trainees before and after the training, and the reduction value of the simulation operation time on the simulator were recorded as well.Results The score of theoretical assessment after training in the observation group was significantly higher than that of the control group(23±8 vs. 11±7, P<0.05). The time required to complete bronchoscopy on the simulator in the control group after was significantly shortened than before the training in the control, which was significantly shorter than the shortened time in the experimental group [(51±31)s vs.(96±26)s, P<0.05], In respect of clinical practice, the success rate of fiber-optic bronchoscopy-guided intubation within 3 minutes in the experimental group was significantly higher than that in the control group(P<0.05).Conclusion High simulation teaching can improve the effect of bronchoscopy training and im
