摘  要:目的 研究小剂量螺旋CT联合迭代重组算法对原发性肝癌Ⅱ期未转移患者图像质量的影响。方法 回顾性分析2017年10月至2020年10月临海市第一人民医院收治的原发性肝癌Ⅱ期未转移患者150例,按照扫描方法将患者分为常规剂量组(n=50)、小剂量组(n=50)、联合组(n=50)。常规剂量组患者行常规剂量螺旋CT扫描,小剂量组患者行小剂量螺旋CT扫描,联合组采用小剂量螺旋CT联合迭代重组算法扫描。结果 比较3组患者的CT图像质量评分、噪声比,差异有统计学意义(P<0.05)。3组患者平扫噪音值、动脉噪音值、静脉噪音值比较,差异无统计学意义(P>0.05)。与常规剂量组相比,小剂量组、联合组的有效剂量和患者所受辐射量较低,差异有统计学意义(P<0.05)。联合组患者扫描图像主观评分高于常规剂量组和小剂量组,图像质量最佳。联合组、常规剂量组小剂量组对原发性肝癌Ⅱ期未转移患者诊断价值较高,差异有统计学意义(P<0.05)。结论 使用小剂量螺旋CT对Ⅱ期未转移原发性肝癌患者进行扫描,可在一定程度上减轻患者扫描所受的辐射量,提高图像质量,对患者的机体损伤较小,值得在临床上应用。Objective To study the effect of low-dose spiral CT combined with iterative recombination algorithm on image quality of patients with stage Ⅱ primary liver cancer without metastasis. Methods A retrospective analysis was performed on 150 patients with stage Ⅱ non-metastatic primary liver cancer who were admitted to Linhai first people’s Hospital from October 2017 to October 2020. According to the scanning method, the patients were randomly divided into the conventional dose group(n=50), the small dose group(n=50), and the combination group(n=50). Patients in the regular dose group were scanned with regular dose spiral CT, while patients in the small dose group were scanned with small dose spiral CT. Patients in the combination group were scanned with small dose spiral CT combined with iterative recombination algorithm. Results Compared with the CT image quality scores and noise ratios of the three groups, the differences were statistically significant(P<0.05),and no statistically significant differences were found between the three groups for the values of unenhanced noise, arterial noise, or venous noise(P>0.05). Compared with the conventional dose group, the effective dose and the amount of radiation received by patients in the low-dose group and the combination group were lower with statistical difference(P<0.05). Patients in the combination group had a higher subjective rating of the scanned images and the best image quality compared with those scanned in the conventional dose or low-dose groups. The low-dose group of combination group and conventional dose group showed high diagnostic value for patients with primary liver cancer stage Ⅱ without metastasis, which was statistically different(P<0.05). Conclusion The use of low-dose spiral CT for the scanning of stage Ⅱ unmetastatic primary liver cancer patients can reduce the amount of radiation received by the scanning of patients to a certain extent, improve the image quality, the body of patients with little damage,worthy of clinical application.
