摘　　要：目的:探究血清可溶性血管内皮生长因子受体-1(sflt-1)及超声动脉血流参数阻力指数(RI)、搏动指数(PI)及收缩期与舒张期峰值流速比(S/D)预测子痫前期(PE)临床价值。方法:2019年8月-2022年5月本院就诊的106例具备PE高危因素的孕妇纳为研究对象,采集孕16~20周外周静脉血,酶联免疫吸附法检测血清sflt-1水平,孕20~24周多普勒超声检测子宫动脉血流参数。确诊PE48例为PE组,血压正常58例为对照组,比较两组血清sflt-1及超声动脉血流参数,绘制受试者工作特征曲线(ROC),分析各指标预测PE价值。结果:PE组确诊时收缩压(SBP)、舒张压(DBP)、血白蛋白、血尿氮素水平均高于对照组,胎盘质量及新生儿出生体重均低于对照组(P<0.05)。PE组血清sflt-1(27.15±3.78 mg/L)、RI(0.92±0.18)、PI(1.52±0.21)、S/D(4.16±1.11)水平均高于对照组(5.49±1.14 mg/L、0.73±0.16、1.01±0.18、3.63±0.79)(P<0.05)。ROC分析,血清sflt-1预测PE价值与RI、PI相似,S/D预测价值最低,RI、PI、S/D联合预测PE的价值高于各参数指标,曲线下面积(AUC)=0.856;血清sflt-1、RI、PI、S/D联合预测效能最高,AUC=0.920。结论:PE患者血清sflt-1及彩色超声多普勒子宫动脉血流参数PI、RI、S/D水平均异常升高,各指标联合应用预测PE的效能较高,可作为临床筛查或预测PE提供参考指标。Objective: To investigate the clinical values of the level of serum soluble vascular endothelial growth factor receptor-1(sflt-1) and the values of the arterial blood flow parameters by ultrasound, such as resistance index(RI), pulsatility index(PI), and systolic to diastolic peak velocity ratio(S/D) for predicting the preeclampsia(PE) of pregnant women. Methods: 106 pregnant women with high-risk factors of PE were enrolled as the research objects. The peripheral venous blood of these women was collected from 16 to 20 gestational weeks for detecting the level of their serum sflt-1 by enzyme-linked immunosorbent assay(ELISA). The uterine artery blood flow parameters of these women during 20-24 gestational weeks were measured by Doppler ultrasound. 48 women with confirmed PE were included in study group, and 58 women with normal blood pressure were included in control group. The serum sflt-1 level and the values of arterial blood flow parameters by ultrasound of the women were compared between the two groups. Receiver operator characteristic(ROC) curve was drawn to analyze the value of the serum sflt-1 level and the values of arterial blood flow parameters of the women for predicting their PE. Results: The values of systolic blood pressure(SBP) and diastolic blood pressure(DBP), and the levels of blood albumin and blood nitrogen of the women at diagnosis in the study group were significantly higher than those of the women in the control group, and the placental mass and the birth weight of newborns in the study group were significantly lower than those of the women in the control group(P<0.05). The serum sflt-1 level(27.15±3.78 mg/L), and the values of RI(0.92±0.18), PI(1.52±0.21), and S/D(4.16±1.11) of the women in the study group were significantly higher than those(5.49±1.14mg/L, 0.73±0.16, 1.01±0.18, and 3.63±0.79) of the women in the group(P<0.05).
