摘 要:目的 探讨不同危险度分层骨髓增生异常综合征(MDS)患者骨髓血髓源性抑制细胞(MDSCs)、外周血T淋巴细胞各亚群及相关免疫抑制因子的表达变化,阐明其在MDS疾病进展中的作用。方法 应用流式细胞术检测60例初治MDS患者(MDS组)和20例缺铁性贫血患者(对照组)骨髓血MDSCs,外周血CD4^(+)T细胞、CD8^(+)T细胞、颗粒酶B及调节性T细胞(Tregs)的表达水平,ELISA法检测免疫抑制因子白介素10(IL-10)和转化生长因子β(TGF-β)的表达水平,同时根据修订的国际预后积分系统(IPSS-R)将MDS患者分为较低危组(L-MDS组)和较高危组(H-MDS组),分析不同分组患者中MDSCs、CD4^(+)T细胞、CD8^(+)T细胞、颗粒酶B及Tregs,免疫抑制因子IL-10及TGF-β的表达差异。结果 与对照组相比,MDS患者MDSCs及Tregs的表达量增高,CD4^(+)T细胞、CD8^(+)T细胞、颗粒酶B表达量显著降低,差异有统计学意义(P<0.05)。在MDS的分组分析中,L-MDS组MDSCs和Tregs表达量低于H-MDS组(P<0.05),L-MDS组和H-MDS组CD8^(+)T细胞表达量无明显差异(P=0.345),而L-MDS组CD4^(+)T细胞和颗粒酶B的表达量高于H-MDS组,差异有统计学意义(P<0.001)。与对照组相比,MDS患者免疫抑制因子IL-10及TGF-β表达水平增高,差异有统计学意义(P=0.015),H-MDS组IL-10及TGF-β水平高于L-MDS组,差异有统计学意义(P<0.001)。结论 MDS患者中MDSCs表达量增加,T细胞亚群表达紊乱,且与疾病危险度分层有关,可能参与了疾病进展。Objective To investigate the expression of myeloid-derived suppressor cells(MDSCs) in bone marrow blood, T lymphocyte subsets in peripheral blood and related immunosuppressive factors of the patients with myelodysplastic syndrome(MDS) stratified in different risk levels, and to elucidate the involvement in MDS disease progression. Methods The expression levels of MDSCs in bone marrow blood, CD4^(+)T cells, CD8^(+)T cells, granitase B and regulatory T cells(Tregs) in peripheral blood of 60 treatment-naive MDS patients and 20 patients with iron deficiency anemia(control group) were detected by flow cytometry. The expression levels of immunosuppressive factor interleukin-10(IL-10) and transforming growth factor β(TGF-β) were detected by ELISA, and the patients were divided into the lower risk group and the higher risk group according to the revised International Prognostic Scoring System(IPSS-R) to analyze the expression differences of MDSCs, CD4^(+)T cells, CD8^(+)T cells, granzyme B and Tregs, immunosuppressive factors IL-10 and TGF-β in different groups of the patients. Results Compared with the control group, the expression of MDSCs and Tregs was increased;and the expression of CD4^(+)T cells, CD8^(+)T cells and granzyme B were significantly decreased in MDS patients, and the difference was statistically significant(P<0. 05).And in the group analysis of MDS, MDSCs and Tregs expression was lower in the L-MDS group than in the H-MDS group(P<0. 05), and CD8^(+)T cell expression was not significantly different between the L-MDS and H-MDS groups(P= 0. 345), while CD4^(+)T cell and granzyme B expression were higher in the lower risk group than in the relatively high risk group, and the difference was statistically significant(P<0. 001).
