摘　　要：目的 探究脂肪量与肥胖相关基因(Fat mass and obesity associated gene, FTO)单核苷酸多态性(Single nucleotide polymorphism, SNP)和冠状动脉性心脏病(Coronary heart disease, CHD)的关联。方法 收集2021年1月-2021年11月新疆医科大学第五附属医院收治的380例CHD患者作为CHD组,选取同时期360例无心血管疾病以及其他严重疾病的健康体检者作为Control组。收集受试者的一般资料,提取两组受试者外周血基因组DNA,检测FTO基因rs9939609及rs1421085多态性,分析不同基因型与CHD患者代谢指标的关系。结果 与Control组比较,CHD组rs9939609(A/T)位点的基因型分布和基因频率差异无统计学意义(P>0.05)。rs1421085(C/T)位点TT、CT、CC基因型在CHD组中分别为189、123、68例,在Control组中分别为193、127、40例,两组间基因型分布(χ^(2)=6.830 2,P=0.032 9)和等位基因频率(χ^(2)=4.867 1,P=0.027 4)差异有统计学意义。与TT基因型患者比较,rs1421085的CT+CC基因型的患者高血压患病率、总胆固醇(Total cholesterol, TC)和小而密低密度脂蛋白(small,dense low density lipoprotein, sd LDL)水平显著增高,差异有统计学意义(均P<0.05)。Logistic回归分析结果显示,高血压患病史、TC、TC以及rs1421085的C等位基因可能是影响CHD发病的独立风险因素(P<0.05)。结论 FTO基因SNPrs1421085多态性与CHD患者的代谢指标存在显著相关性。Objective To investigate the relationship between single nucleotide polymorphism(SNP) of fat mass, obesity associated gene(FTO) and coronary heart disease(CHD). Methods 380 CHD patients admitted to the hospital from January 2021 to November 2021 were enrolled as CHD group. A total of 360 healthy subjects without cardiovascular disease or other serious diseases in the same period were selected as Control group. The general information of the subjects was collected;genomic DNA was extracted from peripheral blood of the two groups;the polymorphisms of FTO gene rs9939609 and rs1421085 were detected;and the relationship between different genotypes and metabolic indexes in patients with CHD was analyzed. Results Compared with Control group, the genotype distribution and gene frequency of rs9939609(A/T) locus in CHD group were not statistically significant(P>0. 05). The genotypes of TT, CT and CC at rs1421085(C/T) locus were 189, 123 and 68 cases in CHD group and 193, 127 and 40 cases in Control group,it were statistically significant in genotype distribution(χ^(2)= 6. 830 2,P= 0. 032 9) and Allele frequency(χ^(2)= 4. 867 1,P= 0. 027 4) between the two groups. Compared with TT genotype patients, the prevalence of hypertension, total cholesterol(TC) and small, dense low density lipoprotein(sdLDL) levels in patients with CT+CC genotype of rs1421085 were significantly increased(all P<0. 05). Logistic regression analysis showed that history of hypertension, TC, TC and C allele of rs1421085 may be independent risk factors for CHD(P<0. 05). Conclusion There is a significant correlation between SNP rs1421085 polymorphisms of FTO gene and metabolic indexes in patients with CHD.
