摘 要:目的:探讨实施行动导向宣教干预门诊结肠造口患者后对患者疾病认知和心理行为反应特征的影响。方法:选取2018年12月-2019年12月某院收治的280例结肠造口患者,按患者入院顺序编号1-280,其中单数编号纳入对照组,实施常规健康宣教,双数编号患者则纳入观察组采取行动导向宣教干预,两组各140例,为期干预6周,观察两组患者认知度、心理反应特征、心理状态、自护能力和生活质量状况。结果:干预后观察组患者认知度问卷评分明显高于对照组(t=7.555,3.736,10.670,11.968;P<0.05);观察组患者肠造口心理社会行为反应量表(OPBRQ)各维度评分明显高于对照组(t=9.660,9.801,15.803,10.408;P<0.05);观察组焦虑自评量表(SAS)、抑郁自评量表(SDS)评分明显低于对照组(t=-5.637,-7.435;P<0.05);观察组自护能力量表(ESCA)各维度评分及总分明显高于对照组(t=6.896,6.147,3.635,8.210,12.228;P<0.05);观察组肠造口生活质量量表(COH-QOL-OQ)各维度评分明显高于对照组(t=3.414,4.075,5.865,6.216;P<0.05)。结论:行动导向宣教干预结肠造口患者能显著提高患者的疾病认知度和增强造口自我管理技巧,从而减少患者心理行为反应和缓解负性情绪,提高造口自护能力,并有助于生活质量改善。Objective:To investigate the effect of action-oriented education intervention on the characteristics of disease cognition and psychological behavior of patients with colostomy.Methods:A total of 280 colostomy patients admitted to our hospital from December 2018 to December 2019 were selected and numbered 1-280 according to the order of admission.Singular numbers were included in the control group for routine health education, and even numbers were included in the observation group for action-oriented education intervention, with 140 cases in each group for 6 weeks.The recognition, psychological response characteristics, psychological state, self-care ability and quality of life of the two groups were observed.Results:After intervention, the questionnaire score of the observation group was significantly higher than that of the control group(t=7.555,3.736,10.670,11.968;P<0.05).The OPBRQ score of the observation group was significantly higher than that of the control group(t=9.660,9.801,15.803,10.408;P<0.05).The scores of self-rating anxiety Scale(SAS)and self-rating depression Scale(SDS)in the observation group were significantly lower than those in the control group(t=-5.637,-7.435;P<0.05).The scores and total scores of ESCA dimensions in the observation group were significantly higher than those in the control group(t=6.896,6.147,3.635,8.210,12.228;P<0.05).The score of each dimension of enterostomy quality of life scale(COH-QOL-OQ)in the observation group was significantly higher than that in the control group(t=3.414,4.075,5.865,6.216;P<0.05).Conclusion:Action-oriented education intervention for colostomy patients can significantly improve patients’ disease awareness and self-management skills of colostomy, thus reducepatients’ psychological and behavioral reactions, alleviate negative emotions, improve the ability of self-care of colostomy, and contribute to the improvement of life quality.
