摘　　要：在国家大力推进医院信息化建设的背景下,远程复诊需求日益增长,已成为患者需求的重要组成部分.远程复诊需求与门诊需求在医疗资源的使用上存在着潜在冲突,医院需要对其有限的医疗资源做合理规划,以平衡线上线下的患者需求,在保证日常门诊服务的同时,尽量满足远程复诊的需求.对此,本文将运用蒙特卡罗仿真方法,分析远程复诊需求对门诊预约服务系统的影响,以最大化期望收益为目标,给出基于医生空闲时间的远程复诊需求接诊阈值与门诊预约调度的优化建议.数值结果显示,对于门诊服务时间差异较大的科室而言,医院采取“初诊在门诊、复诊在线上”的运营模式在成本和收益两方面都更具有优势.With the development of hospital information construction in China,the growing demand for online further consultation has become a part of patients’demand that can not be ignored.There are some potential conflicts in the use of medical resources between the demand for online further consultation and for outpatient service.Therefore,hospitals need to make reasonable planning for their limited medical resources to balance the demand of patients online and offline.While ensuring daily outpatient service,they should also try to meet the needs of online further consultation.Regarding this,we use the Monte Carlo simulation method to analyze the impact of considering the demand of online further consultation on the outpatient appointment service system in this paper.With the goal of maximizing the expected benefit of the hospital,this paper gives the optimization suggestions of the standard of the patients online reception and outpatient appointment scheduling.The numerical results show that,for the medical departments with large differences in outpatient service time,it will lead to more advantages in cost and income if the hospital adopts the operation mode of"initial diagnosis in outpatient service and follow-up online".
