摘  要:目的探讨昼夜节律紊乱对大鼠心肌缺血再灌注后心脏电生理指标的影响。方法选择健康雄性SD大鼠15只,随机分为假手术组、缺血再灌注组和昼夜紊乱组(缺血再灌注+昼夜节律紊乱),每组5只。所有大鼠饲养于12 h:12 h光照黑暗循环的环境中,其中昼夜紊乱组于术前给予24 h持续光照干预1周。缺血再灌注组和昼夜紊乱组建立缺血再灌注模型。再灌注24 h后记录各组大鼠心电图,分析心率变异性[低频功率(LF)、高频功率(HF)和LF/HF比值]、PR间期、QRS波群时限和QT间期,测量左心室有效不应期。结果与假手术组比较,缺血再灌注组PR间期、QRS波群时限、QT间期明显延长,差异有统计学意义(P<0.05,P<0.01);与缺血再灌注组比较,昼夜紊乱组PR间期、QRS波群时限、QT间期明显延长[(55.69±2.11)ms vs(48.73±2.55)ms,(40.45±3.89)ms vs(31.38±5.14)ms,(70.49±4.58)ms vs(60.72±4.87)ms,P<0.01]。与假手术组比较,缺血再灌注组左心室心尖部、左心室心中部和左心室心底部的有效不应期明显缩短,差异有统计学意义(P<0.05,P<0.01);与缺血再灌注组比较,昼夜紊乱组左心室心尖部、左心室心中部和左心室心底部的有效不应期明显缩短,差异有统计学意义(P<0.01)。与假手术组比较,缺血再灌注组LF、LF/HF比值明显升高,HF明显降低,差异有统计学意义(P<0.01);与缺血再灌注组比较,昼夜紊乱组LF、LF/HF比值明显升高,HF明显降低,差异有统计学意义(P<0.05,P<0.01)。结论昼夜节律紊乱可加重缺血再灌注后心电生理的不稳定性。Objective To determine the effects of circadian disruption on cardiac electrophysiological indicators in rats after myocardial ischemia reperfusion(I/R)injury.Methods Fifteen healthy male SD rats were randomly divided into sham operation group,I/R group and circadian disruption group(I/R+circadian disruption),with 5 rats in each group.All rats were raised in a 12-h:12-h light-dark cycle environment,and those from the circadian disruption group were given 24-h continuous light intervention for 1 week before surgery.The I/R model was established in the I/R group and the circadian disruption group.After 24 h of reperfusion,electrocardiography was conducted,and heart rate variability[low frequency power(LF),high frequency power(HF)and LF/HF ratio],PR interval,QRS wave duration and QT interval were analyzed,and left ventricular effective refractory period was measured.Results The PR interval,QT interval and QRS wave duration were significantly prolonged in the I/R group than the sham operation group(P<0.05,P<0.01).Similar results were seen in above indcators in the circadian disruption group than the I/R group(55.69±2.11 ms vs 48.73±2.55 ms,40.45±3.89 ms vs 31.38±5.14 ms,70.49±4.58 ms vs 60.72±4.87 ms,P<0.01).Compared with the sham operation group,the effective refractory periods of the left ventricular apex,left ventricular base and the middle of both were significantly shortened in the I/R group(P<0.05,P<0.01).And these periods were also statistically shortened in the circadian disruption group than the I/R group(P<0.01).The I/R group had notably increased LF and LF/HF ratio,and decreased HF when compared with the sham operation group(P<0.01).The circadian disruption group had even higher LF and LF/HF ratio but lower HF than the I/R group(P<0.05,P<0.01).Conclusion Circadian disruption aggravates the instability of cardiac electrophysiology after myocardial I/R injury.
