针刺联合作业疗法治疗脑卒中后抑郁临床研究

摘　　要：目的:观察针刺联合作业疗法(OT)治疗脑卒中后抑郁(PSD)的临床疗效,以及对5-羟色胺(5-HT)水平的影响。方法:选取120例PSD患者,按随机数字表法分为对照组和联合组各60例。2组均给予西药对症治疗,对照组在此基础上给予OT治疗,联合组在对照组基础上给予针刺治疗,2组均治疗1个月。比较2组治疗前后汉密尔顿抑郁量表(HAMD)-24、美国国立卫生研究院卒中量表(NIHSS)、简明健康状况量表(SF-36)评分及5-HT水平。结果:治疗后,2组HAMD-24、NIHSS评分均较治疗前降低(P<0.05),联合组HAMD-24、NIHSS评分均低于对照组(P<0.05);2组SF-36评分均较治疗前升高(P<0.05),联合组SF-36评分高于对照组(P<0.05)。治疗后,2组5-HT水平均较治疗前升高(P<0.05),联合组5-HT水平高于对照组(P<0.05)。结论:针刺联合OT治疗可有效改善PSD患者的抑郁状态、神经功能和生活质量,提高5-HT水平。Objective:To observe the clinical effect of acupuncture combined with occupational therapy(OT) for post-stroke depression(PSD),and its effects on the level of 5-hydroxytryptamine(5-HT). Methods:A total of 120 cases of PSD patients were selected and divided into the control group and the combination group according to the random number table method, with 60 cases in each group. Both groups were treated with symptomatic treatment of western medicine;the control group was additionally treated with OT,and the combination group was additionally treated with acupuncture based on the treatment of the control group. Both groups were treated for one month. Scores of 24-Item Hamilton Depression Scale(HAMD-24),National Institutes of Health Stroke Scale(NIHSS) and 36-Item Short Form Survey(SF-36),and levels of 5-HT before and after treatment in the two groups were compared. Results: After treatment,the scores of HAMD-24 and NIHSS in the two groups were decreased when compared with those before treatment(P<0.05),and the scores of HAMD-24 and NIHSS in the combination group were lower than those in the control group(P<0.05);SF-36 scores in the two groups were increased when compared with those before treatment(P<0.05),and SF-36 score in the combination group was higher than that in the control group(P<0.05). After treatment, 5-HT levels in the two groups were increased when compared with those before treatment(P<0.05),and the level in the combination group was higher than that in the control group(P<0.05). Conclusion: Acupuncture combined with OT can effectively alleviate the depression status of PSD patients,and improve their neurological function,quality of life and5-HT levels.
