电针疗法联合温针灸疗法治疗脑卒中并发肩手综合征临床研究

摘　　要：目的:观察电针疗法联合温针灸疗法治疗脑卒中并发肩手综合征的临床疗效。方法:将118例脑卒中并发肩手综合征患者随机分为观察组和对照组各59例,对照组采取常规康复治疗,治疗组在对照组的基础上加用电针及温针灸综合治疗。2组均以10 d为1个疗程,共治疗3个疗程。比较2组临床疗效、临床症状体征评分、治疗前后简化Fugl-Meyer运动功能评估量表(FMA)评分、视觉模拟评分量表(VAS)、生活质量量表(QOL)评分。结果:观察组总有效率为93.2%,对照组为76.2%,2组比较,差异有统计学意义(P<0.05)。治疗前,2组僵硬、肿胀、活动受限等症状评分比较,差异无统计学意义(P>0.05);治疗后,2组上述各项症状体征评分均较治疗前降低(P<0.05),且观察组各项评分均低于对照组(P<0.05)。治疗前,2组FMA、VAS、QOL评分比较,差异无统计学意义(P>0.05);治疗后,2组FMA、VAS评分均较治疗前降低(P<0.05),QOL评分均较治疗前升高(P<0.05),且观察组上述各项评分改善较对照组更显著(P<0.05)。结论:电针疗法联合温针灸疗法治疗脑卒中并发肩手综合征患者,可明显提高临床疗效,有效改善临床症状体征,促进患肢康复。Objective: To observe the clinical effect of electroacupuncture therapy combined with warming-needle moxibustion for cerebral apoplexy complicated with shoulder-hand syndrome. Methods:A total of 118 cases of patients with cerebral apoplexy complicated with shoulder-hand syndrome were randomly divided into the observation group and the control group,with 59 cases in each group. The control group was given routine rehabilitation treatment. The treatment group was additionally treated with warming-needle moxibustion combined with electroacupuncture based on the treatment of the control group. Both groups were treated for three courses,with 10 days being a course. The scores of Simplified Fugl-Meyer Motor Function Assessment Scale(FMA),Visual Analogue Scale(VAS) and Quality of Life Scale(QOL) before and after treatment as well as the clinical effects and clinical symptom scores in the two groups were compared. Results:The total effective rate was 93.2% in the observation group and 76.2% in the control group, the difference being significant(P<0.05). Before treatment, there was no significant difference being found in the comparison of the scores of symptoms including stiffness, swelling and activity limitation between the two groups(P>0.05);after treatment, the above symptom scores in the two groups were decreased when compared with those before treatment(P<0.05),and the scores in the observation group were lower than those in the control group(P<0.05). Before treatment,there was no significant difference being found in the comparison of the scores of FMA,VAS and QOL between the two groups(P>0.05). After treatment,the scores of FMA and VAS in the two groups were decreased when compared with those before treatment(P<0.05), and the QOL scores were increased(P<0.05);the improvement of the above scores in the observation group was more significant than that in the control group(P<0.05).
