不同频率电针联合普瑞巴林治疗带状疱疹后遗神经痛的疗效比较

摘　　要：目的观察两种不同频率电针联合普瑞巴林治疗带状疱疹后遗神经痛的临床疗效差异。方法将50例带状疱疹后遗神经痛患者随机分为2/100 Hz组(25例,脱落1例)和2 Hz组(25例)。两组均在口服普瑞巴林基础上行针刺治疗。2/100 Hz组于夹脊穴和阿是穴行疏密波2/100 Hz电针治疗,2 Hz组在相同穴位处行连续波2 Hz电针治疗。观察两组患者治疗前、治疗2周和治疗4周后疼痛视觉模拟量表、医院焦虑抑郁量表、失眠严重指数量表和简明疼痛量表的评分变化。结果与治疗前比较,两组治疗2周和治疗4周后各项评分均降低(P<0.05)。2/100 Hz组治疗4周后疼痛视觉模拟量表和焦虑评分均低于2 Hz组(P<0.05)。结论在口服普瑞巴林治疗基础上,联合电针治疗能减轻带状疱疹后遗神经痛患者的疼痛,改善患者焦虑抑郁状态,提高睡眠质量,且疏密波2/100 Hz的电针治疗在镇痛和改善焦虑情绪方面优于连续波2 Hz电针。Objective To compare the clinical efficacy between two frequencies of electroacupuncture plus Pregabalin in treating postherpetic neuralgia.Method Fifty patients with postherpetic neuralgia were randomly allocated to a 2/100 Hz group(25 cases including 1 dropout case)and a 2 Hz group(25 cases).Acupuncture treatment was offered to both groups in addition to oral Pregabalin.The 2/100 group received electroacupuncture using the 2/100 Hz sparse-dense wave at Jiaji(EX-B1)and Ashi points;the 2 Hz group received electroacupuncture using the 2 Hz continuous wave at the same points.The visual analog scale,hospital anxiety and depression scale,insomnia severity index,and brief pain inventory-short form were observed before treatment and after 2-week and 4-week treatments.Result Compared with the pre-treatment baseline,all measure scores dropped after 2-week and 4-week treatments in both groups(P<0.05).After 4-week treatment,the 2/100 Hz group was lower than the 2 Hz group in comparing the VAS and anxiety scores(P<0.05).Conclusion Based on oral Pregabalin,additional electroacupuncture treatment can ease the pain and improve the anxious and depressive state and sleep quality in patients with postherpetic neuralgia;electroacupuncture with 2/100 Hz sparse-dense wave can produce more significant effects in reducing pain and anxiety than with 2 Hz continuous wave.
