浮针联合药物治疗膝骨关节炎的疗效观察

摘　　要：目的观察浮针联合口服金骨莲胶囊治疗膝骨关节炎的临床疗效及对患者血清基质金属蛋白酶组织抑制因子-1和白细胞介素-1β水平的影响。方法将102例膝骨关节炎患者按入院顺序分为对照组(51例)和研究组(51例)。两组均口服塞来昔布胶囊治疗,对照组另予口服金骨莲胶囊,研究组采用浮针联合口服金骨莲胶囊治疗。比较两组临床疗效,观察两组治疗前后膝关节功能(西安大略麦克马斯特大学骨关节炎指数和Lequesne指数)、疼痛程度(疼痛视觉模拟量表评分)以及血清指标的变化。结果研究组总有效率88.2%,高于对照组的70.6%,差异具有统计学意义(P<0.05)。治疗后,两组疼痛视觉模拟量表评分、西安大略麦克马斯特大学骨关节炎指数和Lequesne指数均低于同组治疗前(P<0.05),两组各项血清指标水平均低于同组治疗前(P<0.05);且研究组治疗后上述各项评分和血清指标均低于对照组(P<0.05)。结论在口服塞来昔布胶囊治疗基础上,浮针联合口服金骨莲胶囊治疗膝骨关节炎可提高临床疗效,改善膝关节功能,减轻疼痛,可能与降低血清基质金属蛋白酶组织抑制因子-1和白细胞介素-1β水平相关。Objective To investigate the clinical efficacy of superficial needling plus oral Jin Gu Lian capsules for knee osteoarthritis and its effect on serum tissue inhibitor of matrix metalloproteinases-1 and interleukin-1βlevels in the patients.Method One hundred and two patients with knee osteoarthritis were allocated,in order of hosptalization,to a control group(51 cases)and a study group(51 cases).Both groups took Celecoxib capsules.Additionally,the control group received oral administration of Jingulian capsules and the study group,superficial needling plus oral administration of Jin Gu Lian capsules.The clinical therapeutic effects were compared between the two groups.Knee functions(the Western Ontario and McMaster Universities osteoarthritis index and the Lequesne index),pain severity(the pain visual analog scale score)and serum indicators were observed in the two groups before and after treatment.Result The total efficacy rate was 88.2%in the study group,which was higher than 70.6%in the control group with a statistically significant difference(P<0.05).After treatment,the pain visual analog scale score,the Western Ontario and Mc Master Universities osteoarthritis index score,the Lequesne index and serum levels of various indicators decreased in the two groups compared with those before treatment(P<0.05)and were lower in the study group than those in the control group(P<0.05).Conclusion On the basis of oral Celecoxib capsules,superficial needling plus oral Jin Gu Lian capsules can heighten the clinical therapeutic effect,improve knee functions and relieve the pain in treating knee osteoarthritis,which may be related to reducing serum tissue inhibitor of matrix metalloproteinases-1 and interleukin-1βlevels.
