摘　　要：目的:探讨一次性包皮切割缝合器对小儿包皮过长和包茎的治疗效果。方法:选取2020年2月-2021年2月在笔者医院行包皮环切术治疗的包皮过长和包茎患儿264例随机分为两组,采用一次性包皮切割缝合器132例患儿为缝合器组,采用传统包皮手术132例为传统组。比较两组手术时间、术中出血量、切口愈合时间、术后疼及治疗费用,记录术后并发症和满意度情况。结果:缝合器组手术时间、术中出血量、愈合时间、疼痛评分均低于传统组,住院费用高于对照组,差异有统计学意义(P<0.05)。缝合器组术后出血发生率高于传统组,术后水肿和感染发生率低于传统组,且并发症总发生率低于传统组,差异有统计学意义(P<0.05)。缝合器组满意度评价高于传统组,差异有统计学意义(P<0.05)。结论:一次性包皮切割缝合器治疗患儿包皮过长和包茎具有手术时间短和术中出血少、疼痛较轻等优势,且并发症发生率较低,虽存在治疗费用高的特点,但可促进术后康复,提高美观度,满意度评价较理想。Objective To observe and analyze the curative effect of disposable foreskin cutting and suturing device for children with excessive prepuce and phimosis.Methods From February 2020 to February 2021,264 children with redundant prepuce and phimosis treated in our hospital were randomly divided into two groups.One hundred and thirty-two children were treated with disposable foreskin cutting and suturing device(the suturing device group),and the other 132 children were treated with traditional prepuce operation(the traditional group).The operation time,intraoperative bleeding,wound healing time,postoperative pain and treatment cost were compared between the two groups,and postoperative complications and satisfaction were recorded.Results The operation time,intraoperative bleeding,healing time and pain score in the suturing device group were lower than those in the traditional group,and the hospitalization cost was higher than that in the control group,the differences were statistically significant(P<0.05).The incidence of postoperative bleeding in the suturing device group was higher than that in the traditional group,the incidence of postoperative edema and infection was lower than that in the traditional group,and the total incidence of complications was lower than that in the traditional group(P<0.05).Satisfaction evaluation of the suturing device group was higher than that of the traditional group(P<0.05).Conclusion Disposable foreskin cutting and suturing device for children with redundant prepuce and phimosis has the advantages of shorter operation time,less bleeding and less pain,and the incidence of surgical complications is low.Although it has the characteristics of high treatment cost,it can promote postoperative rehabilitation and improve the aesthetic appearance,and the evaluation of clinical satisfaction is ideal.
