负压封闭引流联合龙血竭粉在水烫伤足踝创面修复中的应用

摘　　要：目的:分析负压封闭引流技术联合龙血竭粉在水烫伤足踝创面修复中的应用。方法:选择笔者医院2019年7月-2021年1月收治的90例因足踝部水烫伤的患者作为研究对象,依据不同治疗方法均分为两组,对照组45例患者使用龙血竭粉治疗,观察组45例患者在此基础上采取负压封闭引流技术治疗。比较两组临床疗效、创面无渗液时间、结痂时间、创面愈合时间、美国足踝外科学会(American academy of foot and ankle surgery,AOFAS)踝-后足评分、视觉模拟评分表(Visual analogue scale,VAS)、血浆C反应蛋白(C-reactive protein,CRP)水平、白细胞计数(White blood cell count,WBC)及满意评分情况。结果:对照组患者临床疗效(77.78%)显著低于观察组(93.33%),差异有统计学意义(P<0.05)。观察组创面无渗液时间、结痂时间及创面愈合时间均显著低于对照组,差异有统计学意义(P<0.05)。观察组AOFAS评分显著高于对照组,VAS评分显著低于对照组,差异有统计学意义(P<0.05)。观察组WBC、CRP水平显著低于对照组,差异有统计学意义(P<0.05)。对照组患者满意度(77.78%)显著低于观察组(93.33%),差异有统计学意义(P<0.05)。结论:负压封闭引流技术联合龙血竭粉可缩短踝部水烫伤患者创面愈合时间,缓解患者的疼痛,抑制炎症反应,临床疗效较好。Objective To analyze the application of vacuum sealing drainage(VSD)combined with Dragon,s blood powder in repairing water scalded foot and ankle wounds.Methods A total of 90 patients with foot and ankle scald who were admitted to our hospital from July 2019 to January 2021 were selected as the research objects.According to different treatment methods,they were divided into two groups:the control group(45 patients)were treated with Dragon’s blood powder,and the observation group(45 patients)were treated with vacuum sealing drainage on this basis.The clinical efficacy,wound exudation time,scab time,wound healing time,American academy of foot and ankle surgery(AOFAS)ankle-posterior foot score,Visual analog scale(VAS),plasma C-reactive protein(CRP),white blood cell count(WBC)and satisfaction scale were compared between two groups.Results The clinical efficacy of the control group(77.78%)was significantly lower than that of the observation group(93.33%),the difference was statistically significant(P<0.05).The time of no exudation,scab formation and wound healing in the observation group were significantly lower than those in the control group(P<0.05).The AOFAS score of the observation group was significantly higher than that of the control group,and the VAS score was significantly lower than that of the control group(P<0.05).The levels of WBC and CRP in the observation group were significantly lower than those in the control group(P<0.05).The patient,s satisfaction of the control group(77.78%)was significantly lower than that of the observation group(93.33%,P<0.05).Conclusion Vacuum sealing drainage combined with Dragon’s blood powder can shorten the wound healing time,relieve the pain and inhibit the inflammatory response of patients with ankle water scald,with good clinical efficacy.
