病态性肥胖伴糖尿病患者袖状胃切除术治疗无效的相关因素分析

摘　　要：目的分析病态性肥胖伴糖尿病患者袖状胃切除术(LSG)治疗无效的相关因素。方法抽取2020年1月至2021年4月于南阳医学高等专科学校第一附属医院行LSG治疗的病态性肥胖伴糖尿病患者150例。术后随访1年,统计手术效果并分组,设计调查表记录患者一般资料,经Logistic回归分析找出病态性肥胖伴糖尿病患者LSG治疗无效的相关因素。结果术后随访1年,150例病态性肥胖伴糖尿病患者中LSG治疗无效25例,治疗有效125例。无效组患者年龄大于有效组,术前体质指数与术前血清C反应蛋白水平高于有效组(t=5.08、7.65、24.58,P均<0.05),两组其他资料比较差异未见统计学意义(P>0.05)。Logistic回归分析结果显示,年龄大、术前体质指数高、术前血清C反应蛋白水平高是病态性肥胖伴糖尿病患者LSG治疗无效的相关因素(OR>1,P<0.05)。结论LSG治疗病态性肥胖伴糖尿病患者有一定效果,但仍存在治疗无效情况,其相关因素为年龄大、术前体质指数高、术前血清C反应蛋白水平高。Objective To analyze the relevant factors of ineffective laparoscopic sleeve gastrectomy(LSG)in the treatment of morbid obesity complicated by diabetes mellitus.Methods A total of 150 morbid obesity patients with diabetes mellitus who received LSG treatment in the First Affiliated Hospital of Nanyang Medical College from January 2020 to April 2021 were selected.These patients were followed up for 1 year after surgery,and the surgical effects of them were counted,by which patients were grouped.The questionnaire was designed to record the general information of the patients.Logistic regression analysis was used to find out the relevant factors of ineffective LSG in morbid obesity patients with diabetes mellitus.Results After 1-year follow-up,there were 25 cases with ineffective outcome and 125 cases with effective outcome of LSG among 150 morbid obesity patients complicated by diabetes mellitus.The patients in the ineffective group were older than those in the effective group,and the preoperative body mass index and preoperative serum C-reactive protein levels were higher than those in the effective group(t=5.08,7.65,24.58;all P<0.05);and there was no significant difference in other data between the two groups(P>0.05).Results of Logistic regression analysis showed that older age,high preoperative body mass index and high preoperative serum C-reactive protein level were relevant factors for ineffective LSG treatment in morbid obesity patients with diabetes mellitus(OR>1,P<0.05).Conclusions LSG has a certain effect in the treatment of morbid obesity complicated by diabetes mellitus,but some patients still get ineffective outcome.The relevant factors include older age,high preoperative body mass index,and high preoperative serum C-reactive protein level.
