视频眼震电图仪在良性阵发性位置性眩晕诊断中的应用

摘　　要：目的分析视频眼震电图仪(VNG)在良性阵发性位置性眩晕(BPPV)患者诊断中的应用价值。方法抽取2021年10月至2022年3月漯河市中心医院门诊收治的BPPV患者100例,均行Dix-hallpike试验(Dix-hallpike)与滚转试验(Roll Test),均采用VNG和裸眼检查。比较两种检查方式的后半规管BPPV(PSC-BPPV)、前半规管BPPV(ASC-BPPV)、水平半规管BPPV(HSC-BPPV)、混合型BPPV检出率。结果VNG检查对PSC-BPPV典型眼震的检出率(93.15%,68/73)高于裸眼检查(71.23%,52/73),P<0.05;VNG检查对ASC-BPPV典型眼震的检出比例(4/4)与裸眼检查(2/4)比较差异未见统计学意义(P>0.05);VNG检查对HSC-BPPV典型眼震的检出比例(18/19)高于裸眼检查(12/19),P<0.05;VNG检查对混合型BPPV典型眼震的检出比例(4/4)与裸眼检查(2/4)比较差异未见统计学意义(P>0.05)。结论VNG应用于BPPV患者诊断中,可有效观察眼震发生情况,提高眼震检出率。Objective To analyze the application value of videonystagmography(VNG)in diagnosis of benign paroxysmal positional vertigo(BPPV)patients.Methods A total of 100 outpatient BPPV patients in Luohe Central Hospital from October 2021 to March 2022 were selected.Both Dix-hallpike and Roll Test were performed,and VNG and naked eye inspection were performed.The detection rates of BPPV of posterior semicircular canal(PSC-BPPPV),BPPV of anterior semicircular canal(ASC-BPPV),BPPV of horizontal semicircular canal(HSC-BPPV),and mixed BPPV of the two methods were compared.Results The detection rate of typical nystagmus in PSC-BPPV by VNG was 93.15%(68/73),which was higher than the 71.23%(52/73)in naked eye(P<0.05).These was no significant difference in comparison of detection ratio of typical nystagmus in ASC-BPPV by VNG(4/4)and naked eye(2/4),P>0.05.The detection ratio of typical nystagmus in HSC-BPPV by VNG ratio(18/19)was higher than that in naked eye(12/19),P<0.05.There was no significant difference in comparison of detection ratio of typical nystagmus of mixed BPPV by VNG(4/4)and naked eye(2/4),P>0.05.Conclusions VNG can effectively observe the occurrence of nystagmus,and improve the detection rate of nystagmus in patients with BPPV.
