三黄汤湿敷治疗重型颅脑损伤昏迷并发失禁性皮炎的效果观察

摘　　要：目的探讨三黄汤湿敷治疗重型颅脑损伤昏迷并发失禁性皮炎(IAD)的效果。方法抽取2019年5月至2022年5月焦作煤业(集团)有限责任公司中央医院收治的76例重型颅脑损伤昏迷并发IAD患者,使用便利抽样法分成对照组与研究组,每组38例。对照组予以复方炉甘石治疗,观察组予以三黄汤湿敷治疗。比较两组临床疗效及临床指标(红肿面积、愈合时间、住院天数);比较两组治疗前后皮肤状况[IAD皮肤状况评分表(SCAT)]、皮损状况[IAD皮肤损伤评估量表(IADS)]及疼痛状况[视觉模拟评分法(VAS)评分]。结果治疗后,研究组临床总有效率(94.74%,36/38)高于对照组(78.95%,30/38,P<0.05)。治疗前,两组SCAT、IADS及VAS评分比较差异未见统计学意义(P>0.05);治疗后,研究组SCAT、IADS及VAS评分均低于对照组,临床疗效高于对照组,红肿面积小于对照组,愈合时间及住院天数均短于对照组,差异有统计学意义(P<0.05)。结论对重型颅脑损伤昏迷并发失禁性皮炎患者予以三黄汤湿敷治疗效果较好,有利于改善患者皮肤状况,促进皮肤愈合,降低疼痛感,缩短住院时间。Objective To investigate the effect of Sanhuang decoction wet compress on skin wound healing in patients with severe craniocerebral injury and coma complicated by incontinence-associated dermatitis(IAD).Methods Seventy-six patients with severe craniocerebral injury and coma complicated by IAD who were admitted to Central Hospital of Jiaozuo Coal Industry(Group)Co.,Ltd from May 2019 to May 2022 were selected,and they were divided into control group and study group by convenience sampling,with 38 cases in each group.The control group underwent compound calamine treatment,and the observation group was treated by Sanhuang decoction wet compress.The therapeutic effect,clinical indicators(area of redness and swelling,healing time and hospital stay)were compared between the two groups;the preoperative and postoperative skin conditions assessed by IAD skin condition assessment tool(SCAT),skin lesions assessed by IAD severity instrument(IADS)and pain condition assessed by visual analogue scale(VAS)were compared between the two groups.Results After treatment,the total effective rate of the study group(94.74%,36/38)was higher than that of the control group(78.95%,30/38),P<0.05.There was no significant difference in SCAT scores,IADS scores and VAS scores between the two groups before treatment(P>0.05).After treatment,the above scores of the study group were lower than those of the control group,while the area of redness and swelling of the study group was smaller than that of the control group,and the healing time and hospital stay were shorter than those of the control group(P<0.05).Conclusions Sanhuang decoction wet compress is effective in the treatment of patients with severe craniocerebral injury and coma complicated by IAD.It is beneficial to improve the patients’skin condition,promote skin healing,relieve pain,and shorten hospital stay.
