罗哌卡因联合舒芬太尼改善高危妊娠剖宫产术后凝血功能效果观察

摘　　要：目的探讨罗哌卡因联合舒芬太尼改善高危妊娠剖宫产术后凝血功能的效果。方法选取医院2020年8月至2021年3月收治的高危妊娠产妇200例,按随机数字表法分为A1组(罗哌卡因+舒芬太尼)、A2组(罗哌卡因),各100例;选同期医院80例正常妊娠剖宫产分娩产妇,按随机数字表法分为B1组(罗哌卡因+舒芬太尼)、B2组(罗哌卡因),各40例。比较4组患者术前1 d(T_(0))、手术当天(T_(1))、术后第1天(T_(2))、术后第2天(T_(3))时凝血功能指标。结果T_(0)和T_(1)时,A1组、A2组产妇凝血功能指标相近,B1组、B2组患者凝血功能指标相近(P>0.05);A1组、A2组产妇凝血酶时间(TT),活化部分凝血酶时间(APTT),凝血酶原时间(PT)水平较B1组、B2组低,纤维蛋白原(Fib)和D-二聚体(D-D)水平较B1组、B2组高(P<0.05);T_(2)和T_(3)时,4组TT,APTT,PT水平均升高,Fib,D-D水平均降低,其中A1组患者的TT,APTT,PT水平较A2组高,Fib,D-D水平较A2组低(P<0.05);T_(2)和T_(3)时B1组、B2组产妇的上述指标无显著差异(P>0.05)。结论与单用罗哌卡因相比,罗哌卡因联合舒芬太尼可有效改善高危妊娠产妇剖宫产后的凝血功能。Objective To investigate the effect of ropivacaine combined with sufentanil on blood coagulation after cesarean section in high-risk pregnancy.Methods A total of 200 high-risk pregnant women in the hospital from August 2020 to March 2021 were randomly divided into group A1(ropivacaine+sufentanil)and group A2(ropivacaine),with 100 cases in each group.A total of 80 normal pregnant women underwent cesarean section in the hospital during the same period were randomly divided into group B1(ropivacaine+sufentanil)and group B2(ropivacaine),with 40 cases in each group.The blood coagulation function indexes of the four groups were compared on the first day before operation(T_(0)),the day of operation(T_(1)),the first day after operation(T_(2)),and the second day after operation(T_(3)).Results At T_(0) and T_(1),the levels of blood coagulation function in groups A1 and A2 were similar,and those in groups B1 and B2 were similar(P>005).The levels of thrombin time(TT),activated partial thrombin time(APTT)and prothrombin time(PT)in groups A1 and A2 were lower than those in groups B1 and B2,while the levels of fibrinogen(Fib)and D-dimer(D-D)in groups A1 and A2 were higher than those in groups B1 and B2(P<005).At T_(2) and T_(3),the levels of TT,APTT and PT in the four groups increased,and the levels of Fib and D-D decreased.The levels of TT,APTT and PT in group A1 were higher than those in group A2,while the levels of Fib and D-D in groups A1 were lower than those in group A2(P<005).At T_(2) and T_(3),there was no statistical difference between the above indexes in group B1 and group B2(P>005).Conclusion Compared with ropivacaine alone,ropivacaine combined with sufentanil can effectively improve the coagulation function of high-risk pregnant women after cesarean section.
