消肿汤治疗腰椎间盘突出症椎间孔镜髓核摘除术后神经根水肿临床研究

摘　　要：目的探讨消肿汤治疗椎间孔镜腰椎间盘突出症髓核摘除术后减轻神经根水肿的临床疗效。方法选取2019年11月至2021年11月在医院接受治疗的腰椎间盘突出症患者86例,采用双盲法分为观察组和对照组,各43例。所有患者均行椎间孔镜腰椎间盘突出症髓核摘除术,术后观察组患者给予消肿汤治疗,对照组患者给予常规治疗。结果术后5,7 d,两组患者前列腺素E_(2)(PGE_(2))、血清5-羟色胺(5-HT)水平显著降低,且观察组显著低于对照组(P<0.05);两组患者功能障碍指数问卷表(ODI)、视觉模拟评分法(VAS)评分均显著降低,但日本骨科协会评分治疗分数(JOA)评分均显著升高,且观察组显著优于对照组(P<0.05)。结论消肿汤治疗椎间孔镜手术后神经根水肿效果显著,可有效降低疼痛介质对神经根水肿的促进效果,进一步缓解疼痛,改善腰椎及下肢功能障碍。Objective To investigate the clinical efficacy of Xiaozhong Decoction in the treatment of postoperative nerve root edema of patients with lumbar disc herniation undergoing percutaneous transforaminal endoscopic discectomy(PTED).Methods A total of forty-three patients with lumbar disc herniation admitted to the hospital from November 2019 to November 2021 were selected and divided into the observation group and the control group according to the double blind method,with 43 cases in each group.The patients in the two groups were given PTED,on this basis,the patients in the observation group were given postoperative Xiaozhong Decoction,the patients in the control group were given conventional treatment.Results Five and seven days after operation,the levels of prostaglandin E_(2)(PGE_(2))and serum 5-hydroxytryptamine(5-HT)in the two groups were significantly reduced,and the levels of them in the observation group were significantly lower than those in the control group(P<0.05).The scores of Oswestry disability index(QDI)scale and visual analogue scale(VAS)were significantly reduced,but the Japanese Orthopaedic Association(JOA)scores were significantly increased,and the scores of them in the observation group were significantly better than those in the control group(P<0.05).Conclusion Xiaozhong Decoction in the treatment of PTED postoperative nerve root edema has significant efficacy,which can reduce the promoting effect of pain mediator on nerve root edema and relief the pain,and improve the lumbar and lower limb dysfunction.
