吉西他滨膀胱灌注治疗非肌层浸润性膀胱癌临床观察

摘　　要：目的探讨吉西他滨膀胱灌注治疗非肌层浸润性膀胱癌(NMIBC)的临床疗效。方法选取医院2020年7月至2021年7月收治的NMIBC患者60例,按随机数字表法分为对照组和研究组,各30例。患者均予膀胱肿瘤电切术,术后对照组患者灌注药物为表柔比星,研究组患者灌注药物为吉西他滨。结果研究组患者的复发率为13.33%,显著低于对照组的60.00%(P<0.05);研究组患者的不良反应(发热、肉眼血尿、尿路刺激、膀胱区疼痛)发生率均显著低于对照组(P<0.05)。灌注后,研究组患者的T淋巴细胞CD_(4)^(+)水平和CD_(4)^(+)/CD_(8)^(+)均显著高于对照组,CD_(8)^(+)水平显著低于对照组(P<0.05);免疫球蛋白M(IgM)、免疫球蛋白G(IgG)和免疫球蛋白A(IgA)的水平均显著高于对照组(P<0.05)。研究组患者的治疗费用显著低于对照组(P<0.05);研究组患者的生活质量(社会功能、躯体功能、角色功能)评分均显著高于对照组(P<0.05)。结论吉西他滨膀胱灌注治疗NMIBC效果良好,可有效改善患者的免疫功能,降低不良反应发生率和复发率,减少治疗费用,提高生活质量。Objective To investigate the clinical efficacy of gemcitabine intravesical instillation in the treatment of nonmuscular invasive bladder cancer(NMIBC).Methods A total of 60 patients with NMIBC admitted in the hospital from July 2020 to July 2021 were selected and divided into control group and study group according to random number table method,30 cases in each group.All patients were treated with bladder tumor resection.Epirubicin was infused in the control group and gemcitabine was infused in the study group.Results The recurrence rate of patients in the study group was 13.33%,which was significantly lower than 60.00%in the control group(P<0.05).The incidence of adverse drug reactions(fever,gross hematuria,urinary tract irritation,bladder pain)in the study group was significantly lower than that in the control group(P<0.05).After perfusion,the levels of T lymphocyte subsets CD_(4)^(+)and CD_(4)^(+)/CD_(8)^(+)in the study group were significantly higher than those in the control group,while the CD_(8)^(+)level in the study group was significantly lower than that in the control group(P<0.05).The levels of immunoglobulin M(IgM),immunoglobulin G(IgG)and immunoglobulin A(IgA)in the study group were significantly higher than those in the control group(P<0.05).The cost of treatment in the study group was significantly lower than that in the control group(P<0.05).The scores of quality of life(social function,physical function,role function)in the study group were significantly higher than those in the control group(P<0.05).Conclusion Gemcitabine intravesical instillation is effective in the treatment of NMIBC,which can effectively improve the immune function of patients,reduce the incidence of adverse reactions and recurrence rates,reduce the cost of treatment,and improve the quality of life.
