摘　　要：目的研究亚临床甲状腺功能减退(SCH)与接受经皮冠状动脉介入治疗(PCI)的老年冠心病病人死亡的相关性。方法回顾性分析1665例行PCI治疗的65岁以上老年冠心病病人,随访4年,随访内容为出院后服药情况及死亡情况。结果最终纳入SCH病人166例(9.97%),甲状腺功能正常(ET)病人1499例(90.03%)。共发生46例死亡,死亡率为2.76%,SCH组和ET组病人4年累积全因死亡率分别为12.65%和1.67%。Kaplan-Meier分析显示,SCH组病人的全因死亡率显著高于ET组。Cox比例风险回归模型分析显示,年龄、高血压、糖尿病、吸烟、左主干病变及SCH为行PCI治疗的老年冠心病病人死亡的独立危险因素。结论SCH为老年PCI术后病人死亡风险的独立危险因素。Objective To investigate the association between subclinical hypothyroidism(SCH)and death in the elderly patients with coronary artery disease(CAD)afteRpercutaneous coronary intervention(PCI).Methods A total of 1665 elderly CAD patients aged≥65 years old who underwent PCI were retrospectively analyzed.All patients received a follow-up foRthe mortality and medication afteRdischarge foR4 years.Results The final cohort included 166 patients with SCH(9.97%)and 1499 patients(90.03%)with normal thyroid function(ET).A total of 46 patients died during the follow-up period,and the mortality rate was 2.76%.The 4-yeaRcumulative all-cause mortality in SCH group and ET group was 12.65%and 1.67%,respectively.Kaplan-MeieRanalysis showed that all-cause mortality in SCH group was significantly higheRthan that in ET group.Cox proportional regression model analysis showed that age,hypertension,diabetes,smoking,left main coronary artery disease and SCH were the independent risk factors foRdeath of the elderly CAD patients undergoing PCI.Conclusions SCH is an independent risk factoRfoRthe mortality of the elderly patients afteRPCI.
