摘 要:目的:探讨超微血流成像技术(superb micro-vascular imaging,SMI)与类风湿关节炎(rheumatoid arthritis,RA)辨证分型的关系,研究SMI在类风湿关节炎中的诊断价值。方法:选取2021年1月—2022年5月于广州中医药大学第一附属医院就诊的类风湿关节炎患者75例,根据辨证分型分为湿热痹阻证组20例、寒湿痹阻证组25例、肝肾亏虚组30例,对腕关节滑膜进行超声检查,获取滑膜厚度、关节积液、血流信号、骨侵蚀病变参数,并用SMI技术半定量评价与定量检测增生滑膜血管指数(VI)。结果:湿热痹阻证组滑膜增生厚度、血流指标检出情况均高于寒湿痹阻证组、肝肾亏虚组,差异具有统计学意义(P<0.05)。SMI定量检测中,湿热痹阻、寒湿痹阻、肝肾亏虚增生滑膜VI值呈递减趋势,且组间与组内比较差异均具有统计学意义(P<0.05)。结论:SMI技术可以敏感地实时检测微血流信号,在RA辨证分型及疾病监测中具有一定的作用。Objective To investigate the relationship between superb micro-vascular imaging(SMI) and the dialectical classification of rheumatoid arthritis(RA),and to study the diagnostic value of SMI in rheumatoid arthritis.Methods From January 2021 to May 2022,75 patients with rheumatoid arthritis who were treated in the First Affiliated Hospital of Guangzhou University of Chinese Medicine were selected,according to syndrome differentiation and classification,75 patients diagnosed clinically were divided into damp-heat obstruction syndrome group(20 cases),cold-damp obstruction syndrome group(25 cases),and liver kidney deficiency group(30 cases).The wrist synovium was examined by ultrasound to obtain synovium thickness,fluid accumulation depth,blood flow signal,bone erosion lesion parameters,and semi quantitative evaluation and quantitative detection of proliferative synovium vascular index(Ⅵ) with SMI technology.Results The synovial hyperplasia thickness and blood flow indexes of the damp heat obstruction group were higher than those in the cold and wet paralysis obstruction group and the liver and kidney deficiency group,and the difference was statistically significant(P <0.05).In the quantitative detection of SMI,the value of VI of synovium of damp heat arthralgia,cold damp arthralgia,liver and kidney deficiency hyperplasia showed a decreasing trend, and there was statistical significance between groups and within groups(P <0.05).Conclusion SMI technology can detect micro blood flow signals sensitively and in real time,which plays a certain role in RA dialectical typing and disease monitoring.
