摘　　要：目的:探讨卵巢子宫内膜异位囊肿的CT表现及误诊原因,提高诊断准确性。方法:选取2015年4月—2020年5月在唐山市妇幼保健院经手术及病理证实为卵巢子宫内膜异位囊肿的患者共320例,均行CT平扫及增强检查,分析其CT图像特征。结果:320例患者共418个病灶,其中位于右侧卵巢212个,左侧卵巢206个;囊肿大小0.8 cm×1.0 cm~8.8 cm×19.0 cm;单囊77个,双囊35个,“卫星”囊95个,多囊210个,囊中囊2个;类圆形或类卵圆形188个,不规则形68个,卵圆形136个,圆形12个,葫芦状14个;囊内密度不均336个,密度均匀82个;薄壁62个,壁厚薄不均356个;盆腔粘连者304例,其中重度粘连11例。结论:多囊或“卫星”囊、囊内密度较高且密度不均、囊壁较厚且厚薄不一、同侧卵巢显示不清、盆腔粘连是卵巢子宫内膜异位囊肿特征性的CT表现,CT对卵巢子宫内膜异位囊肿的诊断有重要价值。Objective To explore the CT manifestations of ovarian endometriotic cyst and the causes of misdiagnosis,to improve the accuracy in the diagnosis of endometriotic cyst.Methods 320 patients with ovarian endometriotic cysts confirmed by surgery and pathology in Tangshan Maternal and Child Health Hospital from April 2015 to May 2020 were selected,all of whom underwent plain CT and enhanced CT examination to analyze their CT image features.Results Among the 320 patients(418 lesions),212 lesions were locate at the right adnexal area,206 lesions were locate at the left adnexal area.The cysts were 0.8 cm×1.0 cm to 8.8 cm×19.0 mm in size and monolocular cysts in 77 lesions,dual-locular cysts in 35 lesions,"satellite"locular cysts in 95 lesions,multilocular cysts in 210 lesions,locular daughter cysts 2 lesions.Among the 320 lesions,there were188 lesions showed resemble round or oval,68 lesions showed slightly irregular shape,there were 136 lesions showed oval,12 lesions showed round,14 lesions showed gourd-shaped;336 cysts fluid was inhomogeneous density,82 cysts fluid was homogeneous density;62 lesions showed thin wall,356 lesions showed the thick and thin wall;304 patients showed pelvic adhesions,11 of them showed serious adhesion.Conclusion The specific CT manifestations features of endometriosis cyst were"satellite"locular and multiple-locular,high density and uneven density in the cysts,showed the thick and thin wall,Ipsilateral ovary showed not clear and showed pelvic adhesions,the CT findings of ovarian endometriotic cyst is very important for diagnose.
