摘 要:目的探究白细胞介素-22(IL-22)及其相关CD4+T细胞亚群在斑秃患者外周血及皮损中的表达。方法抽取2018年10月至2021年10月在山西省人民医院皮肤性病科就诊的91例斑秃患者为观察组,抽取同期于皮肤性病科及神经外科住院的80例非斑秃患者为对照组,检测两组患者血清IL-22、白细胞介素-23(IL-23)、白细胞介素-17(IL-17)和干扰素-γ(IFN-γ)水平,CD4+IFN-γ+辅助性T细胞1(Th1)、CD4+IL-17+辅助性T细胞17(Th17)和CD4+IL-22+辅助性T细胞22(Th22)占比,检测皮损组织中IL-22表达量,IL-22、信号转导与转录激活因子(STAT)3、磷酸化-STAT3(p-STAT3)的蛋白水平以及IL-22、STAT3的mRNA水平。结果观察组血清IL-22、IL-23、IL-17、IFN-γ水平及CD4+IFN-γ+Th1、CD4+IL-17+Th17和CD4+IL-22+Th22占比均高于对照组(P均<0.05);观察组头皮组织IL-22表达、IL-22蛋白、STAT3蛋白、p-STAT3蛋白及IL-22 mRNA、STAT3 mRNA水平均高于对照组(P均<0.05);重型斑秃患者血清IL-22、IL-23、IL-17、IFN-γ水平及CD4+IFN-γ+Th1、CD4+IL-17+Th17和CD4+IL-22+Th22占比均高于局限性斑秃患者(P均<0.05);重型斑秃患者皮损组织IL-22表达、IL-22蛋白、STAT3蛋白、p-STAT3蛋白及IL-22 mRNA、STAT3 mRNA水平均高于局限性斑秃患者(P均<0.05);向前-条件方法行Logistic有序逐步回归分析结果显示,IFN-γ、CD4+IL-22+Th22是影响斑秃患者病情的危险因素(P<0.05)。结论斑秃患者外周以及皮损组织中IL-22水平表达升高,其机制可能是通过Janus激酶/STAT3通路介导。Objective To investigate the expression and significance of interleukin-22(IL-22)and cluster of differentiation(CD)4+T cell subsets in peripheral blood and skin lesions of patients with alopecia areata(AA).Methods A total of 91 patients with AA treated in the Department of Dermatology and Venereology of Shanxi Provincial People’s Hospital from October 2018 to October 2021 were selected as the observation group.Eighty patients without AA hospitalized in the Department of Dermatology and Venereology and Department of Neurosurgery at the same time were selected as the control group.The levels of serum IL-22,interleukin-23(IL-23),interleukin-17(IL-17)and interferon-γ(IFN-γ),the proportions of CD4+IFN-γ+T helper(Th)1 cells,CD4+IL-17+Th17 cells and CD4+IL-22+Th22 cells,the expression level of IL-22 in skin lesions,the levels of IL-22 protein,signal transducer and activator of transcription(STAT)3 protein and phosphorylated-STAT3(p-STAT3)protein,and the levels of IL-22 mRNA and STAT3 mRNA in the two groups were detected.Results Levels of serum IL-22,IL-23,IL-17 and IFN-γ,the proportions of CD4+IFN-γ+Th1 cells,CD4+IL-17+Th17 cells and CD4+IL-22+Th22 cells in the observation group,were higher than those in the control group(all P<0.05).The levels of IL-22 expression,IL-22 protein,STAT3 protein,p-STAT3 protein,IL-22 mRNA and STAT3 mRNA in scalp tissues of the observation group were higher than those of the control group(P<0.05).Levels of serum IL-22 expression,IL-23,IL-17 and IFN-γ,the proportions of CD4+IFN-γ+Th1 cells,CD4+IL-17+Th17 cells and CD4+IL-22+Th22 cells in patients with severe AA were higher than those in patients with localized AA(all P<0.05).The levels of IL-22 expression,IL-22 protein,STAT3 protein,p-STAT3 protein,IL-22 mRNA and STAT3 mRNA in patients with severe AA were higher than those in patients with localized AA(all P<0.05).Rsults of Logistic stepwise regression analysis showed that IFN-γand CD4+IL-22+Th22 cells were main risk factors of AA(P<0.05).Conclusions The expression of IL-22 is incre
