摘　　要：目的探究肺表面活性物质(PS)联合经鼻持续气道正压通气(NCPAP)治疗新生儿呼吸窘迫综合征(NRDS)的临床效果。方法选取2017年1月至2021年9月徐州市第一人民医院新生儿室收治的90例NRDS患儿进行回顾性分析。根据治疗方法分为三组:对照组A(NCPAP辅助通气治疗)、对照组B(PS治疗)、研究组C(NCPAP联合PS治疗),每组30例。比较治疗前、治疗1 h后、治疗24 h后三组的pH值、动脉血氧分压(PaO2)、动脉血二氧化碳分压(PaCO_(2))。结果治疗1 h后三组的pH、PaCO_(2)、PaO2比较,差异有统计学意义(P<0.05)。结论牛肺PS联合NCPAP治疗NRDS,可于治疗黄金期(治疗1 h后)改善患儿的氧合,为各脏器的血流灌注赢得宝贵时间,值得新生儿科医师借鉴。Objective To explore the clinical effect of Pulmonary Surfactant(PS)combined with nasal continuous positive airway pressure(NCPAP)in the treatment of neonatal respiratory distress syndrome(NRDS).Methods A total of 90 cases of NRDS admitted to the Department of Pediatrics of Xuzhou First People’s Hospital from January 2017 to September 2021 were retrospectively analyzed.According to the treatment received,they were divided into three groups:control group A(received NCPAP treatment),control group B(received PS treatment),and study group C(received PS combined with NCPAP treatment).30 cases in each group.The changes of pH value,arterial partial pressure of oxygen(PaO 2),arterial partial pressure of carbon dioxide(PaCO 2)in the three groups were compared before treatment,after 1 h and 24 h of treatment.Results After 1 hour of treatment,the differences of pH,PaCO 2 and PaO 2 among the three groups were statistically significant(P<0.05).Conclusion The application of PS combined with NCPAP in the treatment of NRDS can improve the oxygenation of children in the golden period(after 1 h of treatment),and win valuable time for blood perfusion of organs,which is worthy of reference for neonatal physicians.
