摘　　要：目的探讨当归桂枝助阳方联合针刺荥输、下合穴对糖尿病周围神经病变(DPN)阳虚寒凝证患者神经传导速度的影响。方法选取2020年6月至2021年12月本溪市中医院收治的70例阳虚寒凝证DPN患者作为研究对象,采用随机数字表法将其分为对照组与试验组,各35例。两组患者在给予常规治疗基础上,对照组采用针刺荥输、下合穴治疗,试验组用当归桂枝助阳方联合针刺荥输、下合穴治疗。比较两组患者的临床疗效、中医证候积分、神经症状评分与神经传导速度变化。结果试验组的治疗总有效率高于对照组,差异有统计学意义(P<0.05);试验组治疗后中医证候积分与多伦多神经症状(TCSS)评分低于对照组,差异有统计学意义(P<0.05);试验组治疗后神经传导速度高于对照组,差异有统计学意义(P<0.05)。结论采用当归桂枝助阳方联合针刺荥输、下合穴与单一使用针刺治疗DPN阳虚寒凝证患者相比,前者可显著提升临床疗效、改善神经症状与提升神经传导速度。Objective To investigate the influence of Danggui Guizhi Zhuyang Decoction combined with acupuncture at ying-spring and shu-stream point and lower confluent point on nerve conduction velocity in patients with diabetic peripheral neuropathy(DPN)of yang deficiency and cold coagulation syndrome.Methods A total of 70 patients with DPN of yang deficiency and cold coagulation syndrome were admitted to Benxi Hospital of Traditional Chinese Medicine from June 2020 to December 2021 were selected as the research subjects,and they were divided into control group and experimental group by random number table method,with 35 cases in each group.After basic treatment,patients in the control group were treated with acupuncture at ying-spring and shu-stream point and lower confluent point,while patients in the experimental group were treated with Danggui Guizhi Zhuyang Decoction combined with acupuncture at ying-spring and shu-stream point and lower confluent point.The clinical efficacy,traditional Chinese medicine syndrome score,nerve symptom score and nerve conduction velocity were compared between the two groups.Results The total effective rate in the experimental group was higher than that in the control group,and the difference was statistically significant(P<0.05).After treatment,traditional Chinese medicine symptom scores and the Toronto clinical neuropathy scoring system(TCSS)scores in the experimental group were lower than those in the control group,and the differences were statistically significant(P<0.05).The nerve conduction velocity in the experimental group was higher than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Compared with acupuncture alone,Danggui Guizhi Zhuyang Decoction combined with acupuncture at ying-spring and shu-stream point and lower confluent point can significantly improve the clinical effect in the treatment of patients with DPN of yang deficiency and cold coagulation syndrome,improve neurological symptoms and increase nerve conduction velocity in
