甘草附子汤联合温针灸治疗早期强直性脊柱炎疗效的磁共振评价及对血清白介素-35的影响

摘　　要：目的:探析甘草附子汤联合温针灸治疗早期强直性脊柱炎疗效的磁共振(MRI)评价及对血清白介素(IL)-35的影响。方法:选择2019年1月—2021年10月收治的早期强直性脊柱炎患者60例,均行甘草附子汤联合温针灸治疗。对比患者治疗前后的临床量表评分、疼痛评分、MRI评分与血清学指标变化,并分析MRI评分与IL-35的相关性。结果:治疗后,患者的日常生活活动量表(BI)评分明显高于治疗前,中医症候评分与疼痛评分均低于治疗前,差异有统计学意义(P<0.05)。治疗后患者MRI评分中的累及范围计分、水肿强度计分与水肿深度计分及IL-17,IL-35均低于治疗前,差异有统计学意义(P<0.05)。治疗过程中并无严重不良反应出现。相关性分析显示MRI评分与IL-35呈正相关。结论:MRI评分与IL-35能够有效评估早期强直性脊柱炎的疗效情况。甘草附子汤联合温针灸治疗早期强直性脊柱炎的疗效显著,可有效改善患者临床症状,提高日常生活活动能力,利于缓解患者疼痛程度与炎症水平。Objective:To explore the MRI evaluation of the efficacy of gancao fuzi decoction combined with warm acupuncture in the treatment of early ankylosing spondylitis and its effect on serum IL-35.Methods:A total of 60 patients with early ankylosing spondylitis who were treated in hospital from January 2019 to October 2021 were included in the study,and all patients received gancao fuzi decoction combined with warm acupuncture.The clinical scale scores,pain scores,magnetic resonance(MRI)scores and serological indexes of patients before and after treatment were compared,and the correlation between MRI scores and interleukin-35(IL-35)was analyzed.Results:After treatment,the patient's daily living activity scale(BI)score was significantly higher than before treatment,and the TCM syndrome score and pain score were lower than those before treatment,and the difference was statistically significant(P<0.05).After treatment,the MRI scores of the involved range score,edema intensity score and edema depth score were lower than those before treatment(P<0.05).Compared with before treatment,the levels of IL-17 and IL-35 in patients after treatment were lower(P<0.05).No serious adverse reactions occurred during the treatment.Correlation analysis showed that MRI scores were positively correlated with IL-35.Conclusion:MRI score and IL-35 can effectively evaluate the curative effect of early ankylosing spondylitis,and gancao fuzi decoction combined with warm acupuncture has significant curative effect in the treatment of early ankylosing spondylitis,can effectively improve the clinical symptoms of patients,improve the activities of daily living,and It is beneficial to relieve the pain level and inflammation level of patients.
